Objective1.To research the the application of ARFI(Acoustic Radiation Force Impulse)for liver fibrosis assessment.2.Use of ARFI technology to determine the elasticity of the liver tissue,as compared to percutaneous liver biopsy(LB) in patients with chronic hepatitis,and assess the correlation between them.At the same time,to calculate the cutoffs that predicted each degree of fibrosis.Provide a new,noninvasive method for liver fibrosis assessment.Materials and MethodFrom July 2010 to March 2011,use a Siemens ultrasound machine with ARFI technique,to exam the right lobe of liver in 101 cases. The parameter of Virtual Tough Tissues Quantification (VTQ) based on ARFI technique was achieved in all cases and compared in each other.Pathological staging of liver fibrosis according to the Meeting in XIAN in 2000 established diagnostic criteria for chronic hepatitis B. SO of 21 cases (including 20 healthy volunteers and pathology of liver fibrosis SO one case),S1 of the 19,S2 of 21,S3 of 21 cases,S4 of 19 cases,a total of 101 cases.RESULT1,Between the parameter of VTQ and the degree of liver fibrosis,there is a direct,strong,linear correlation (Speaman r=0.902).As the severity of hepatic fibrosis,the elasticity increased.2,With pathologic dianosis as the golden standard,to determine the cutoffs that predicted each degree of fibrosis, we used the ROC curve method.Chose the maximal that the sum of sensitivity and specificity as the cutoffs:1.20m/s(for the prediction of stages S≥1),1.42m/s (S≥2), 1.65m/s(S≥3),1.96m/s(S4)。In a short,every cases in this study that had a result with a value<1.96m/s did not have cirrhosis.3,There was no significantly relation between SO and S1,maybe there have no obvious fibrosis in S1 itself.Conclusion1,In chronic viral hepatitis, it is important that the knowledge of the stage of liver fibrosis for prognosis and for decisions about antiviral treatment.ARFI technique is a noninvasive and reliable method for assessment of liver fibrosis.It can quantify the elastic stiffness of liver tissue,and have a important value in diagnosis of liver fibrosis.2,As the severity of hepatic fibrosis,elasticity increased,ARFI technique can be intuitive,fast response from the liver lesions, to facilitate choice of treatment and the prognosis.
