[Objective] To establish rat model of partial hepatectomy (PH) with independent liver lobe congestion and investigate the effect of liver congestion on liver regeneration and liver function after partial hepatectomy in rats of early stage.[Methods]Partl:To establish rat model of partial hepatectomy (PH) with independent liver lobe congestion:The experiment was carried out to research rat liver anatomy pre-study, familiar with the anatomy of rat livers, and establish the model of independent hepatic congestion.Part2:To investigate the effect of liver congestion on liver regeneration and liver function after partial hepatectomy in rats of early stage.96 healthy aldut male Wistar rats were divided into four groups:Group A1 were performed with 70% PH and Caudate lobe congestion, group A2 were performed with 70% PH, Group B1 were performed with 50% PH and right middle lobe congestion, group B2 were performed with 50% PH.Rats in each group after 1 days,2 days,3 days and 5 days were randomly sacrificed 6. Blood samples (5ml) were obtained from inferior vena cava of rat when rats were sacrificed after anesthesia.Get serum after standing and centrifugalization,then put serum in the refrigerator(-80℃),detect hepatocyte growth factor (HGF) and TNF-aby enzyme-linked immunosorbent assay (ELISA); Record rat weight, liver wet weight, the calculation of liver regeneration rate, make single-cell suspension from part of a liver by mechanical method; Detect Liver cell proliferation index by flow cytometer, part of a liver cryopreserved-80℃, and the remaining part of liver was fixed by formaldehyde, make conventional HE staining and immunohistochemical staining (liver cells proliferating nuclear antigen PCNA), Record PCNA expression. The results of group A1 and group A2,group B1 and group B2 were analyzed using SPSS 17.0 statistical analysis software, observe compa rison of various indicators of group differences in liver regeneration (independent samples t test).[Results]Partl:The model of partial hepatectomy (PH) with independent liver lobe congestion was stable;Part2:Pathological changes:congestion zone of the experimental group at the next day occurred focal necrosis, some areas necrosis and fibrous tissue proliferation occurred, most of hydropic degeneration of liver cells, after the fifth day of non-congestion areas returned to normal liver tissue.Liver function changes:ALT of experimental group (group A1,group A2) were higher than the control group (group B1,group B2) in the early period(1 day,2 day,3 day,5day);Liver regeneration:The liver regeneration index gradually increased, there were significant differences of right lobe liver regeneration index (p<0.01) between experimental group and control group on the first,second,third and fifth day after surgery,and there were no differences of remnant liver regeneration index (p>0.05) between experimental group and control group on the first,second,third and fifth day after surgery;The Change of PCNA and S phase liver cells proportion is consistent, there were significant differences (p<0.01) between experimental group and control group on the first,second,third day after surgery,and there were no differences (p>0.05) between experimental group and control group on the fifth day after surgery; The HGF level in each group reached a peak at the first day after surgery, there were significant differences (p<0.01) between experimental group and control group on the first,second,third day after surgery,and there were no differences (p>0.05) between experimental group and control group on the fifth day after surgery; The TNF-a level of liver tissue in each group reached a peak at the second day after surgery, there were significant differences (p<0.01) between experimental group and control group on the first,second,third day after surgery,and there were no differences (p>0.05) between experimental group and control group on the fifth day after surgery.[Conclusions]1.The rat model of partial hepatectomy with independent liver lobe congestion is stable and could be used for related research.2.Local congestion to some extent increase the liver damage at the early stage, this experiment proved that the congestion of liver have no effect on the liver regeneration at different levels.3. The congestion zone is not complete infarcted region maybe has important clinical significance,especially for those small remnant liver and small graft,the partial liver function in the congestion zone is important to maintain the patient's physiological function,and creat time fore the non-congestion zone to regeneration.
