Objective:The defect of Beta cell function and insulin resistance are two basic links to pathogenesis patients in type 2 diabetes mellitus.(T2DM) The secretion function of langerhans Beta cell of The newly diagnosed T2DM approximately for normal glucose tolerance 50%. The quality and quantity of insulin secretion drop for the patients of T2DM, the ratio of proinsulin to immunoreactive insulin increase for the patients of T2DM, while the quantity of true insulin Accordingly reduces. With devoment of single colne anti-body technique.methods of detect sepciel insulin is more,which detect as true insulin.To observe characteristic changes that quality and quantity of insulin secretion and the ratios of true insulin to immunoreactive insulin in fasting,30minutes120minutes of newly diagnosed patients of type 2 diabetes mellitus around treatment of Xiaoke Pills or repaglinide. To observe the effects to true insulin secretion around treatment of Xiaoke Pills or repaglinide.Methods:The newly diagnosed T2DM in 90 patients divide in two groups in a randomized way. One included 44 patients,which defined as Xiaoke pills group,regulating take Xiaoke pills for seven days; another defined as repaglinide group of 46 patients, which dose repaglinide for seven days; Collecting fasting,30 min and 120 min venous blood around the treatment, blood glucose was measured with glucose oxidase method, true insulin with CLIA and immunoreactive insulin with radioimmunoassay. The ratios of true insulin to immunoreactive insulin were calculated various times. Statistical software of SPSS17.0 was applicated to analyze data. Measurement data was said to x±s, the data of around treatment Xiaoke pills or repaglinide was test by pair T test, P<0.05 means that difference have statistical significance. Two independent sample t-test was applicated to analyze data of control group.of Xiaoke pills and repaglinide, P< 0.05 means that difference have statistical significance.Results:Blood glucose Xiaoke pills group fall (3.4±1.6) mmol/L in Omin, which compared prior treatment; repaglinide group fall (4.4±1.4)mmol/L compared prior treatment respectively in 120min.The quantity of TI secretion of Xiaoke pills group have a distinctly improvement after treatments in 120min (P<0.05). The quantity of TI secretion of repaglinide group have a distinctly improvement.after treatments in 30min (P<0.05).There is a decrease of amplification of Xiaoke pills groug in 30min,which compare to repaglinide group.TI/IRI of Xiaoke Pills group was 0.6O±0.13 in 120minutes, which compared prior treatment to have a obvious increase(P<0.05). TI/IRI of repaglinide group was 0.63±0.08 in 30minutes, which compared prior treatment to have a obvious increase, the changes have statistics significance. With TI calculate early phase insulin secretion index (△Ins30/△G30), there is a increase after treatment of Xiaoke Pills, but increased not significant. there is a increase after treatment of repaglinide, a statistically significant difference.Conclusions:The quantity of TI,IRI and ratios of TI/IRI are increased in 30min and 120min after treatment of Xiaoke Pills or repaglinide. repaglinide show that improvement of early phase of insulin secretion. The quantity of TIand ratios of TI/IRI are increased after treatment of Xiaoke Pills,which have a statistically significant.
