Objective:To explore the relationship between the insulin-like growth factor-1(IGF-I),insulin resistance and prostate volume.Methods:Ninety old patients with benign prostatic hyperplasia were divided into two groups according to lower urinary tract symptoms(LUTS),International Prostate Symptoms Score(IPSS),prostate specific antigen(PSA) and prostate volume which was measured by abdominal ultrasound:experimental group and control group, common data and biochemical characteristics were collected. Serum IGF-I level was detected by ELISA assay.Results:1. Fasting blood glucose [(6.29±0.77) vs (4.77+0.33)mmol/L,fasting serum insulin[(12.92±3.81)vs(8.49±2.04)mU/L],HOMA-IR [(3.60±1.27) vs (1.89±0.36)] and LDL-cholesterol [(3.74±0.37) vs (2.05±0.48) mmol/L] were significantly higher(P<0.05) and IGF-I [(104.63±26.14) vs (169.78±53.70)ug/L], HDL-cholesterol [(1.12±0.20) vs (1.75±0.21) mmol/L] were significantly lower(P<0.01) in experimental group as compared to controls.2. Partial correlation analysis showed that the prostate volume was positively correlated with age,FBG,FINS,HOMA-IR and PSA(P<0.05), while negatively correlated with HDL-C(P<0.01); serum IGF-I level was negatively correlated with age,FBG,FINS,HOMA-IR and LDL-C.Conclusions:1. The index comparison of the two groups:FBG,FINS,HOMA-IR,LDL-C and PSA were significantly higher and IGF-I,HDL-C were significantly lower in experimental group as compared to controls. It showed that the experimental group have serious lower urinary tract symptoms and enlarged prostate volume accompanied with low serum IGF-I level and obvious insulin resistance and dyslipidemia.2. The serum IGF-I level was negatively correlated with age,FBG,FINS,HOMA-IR and LDL-C in the two groups. It showed that IGF-I was related to glycometabolism and lipidmetabolism, which could reflects the level of the insulin resistance.3. The paper showed that the low level of IGF-I accompanied with obvious insulin resistance, and hyperinsulinemia was positively correlated with prostate volume. IGF-I and insulin resistance were also risk factors of BPH and maybe participated in the development of BPH.
