Objective To study the MVD and the expression of VEGF in benign prostatic hyperplasia (BPH), and analyze the effects of finasteride on VEGF and MVD in BPH.Methods Immunohistochemical staining (S-P) was used to examine MVD and the expression of VEGF in 37 cases of BPH and 6 cases of normal prostate (NP). BPH cases was divided into 2 groups according to on finasteride treatment or not. Immunohistochemical parameters were compared in both groups, and chisq test, student t test, Spearman rank correlation test and Wilcoxon test were used for data statistic analysis.Results 1. VEGF expression was negative in all 6 NP; in BPH, the expression of VEGF were negative in 8 cases (21.6%), (+)in 20 cases(54.1%)and (++) in 9 cases(24.3%), respectively with significant difference statistically (P?0.05). MVD parameter was 4.83?.83 in NP and 8.00?.28 in BPH, statistic significance existing between the two groups (P<0.05).2. In BPH, the expression of VEGF correlated to MVD (P<0.05),There was no correlation between the size of prostate and MVD and the expression of both VEGF in BPH(P>0.05).3. In BPH, the expression of VEGF in the 19 patients treated with finasteride was (-) in 6 cases(31.6%), (+) in 13 cases(68.4%), respectively. In the 18 cases without finasteride, VEGF was (-) in 2 cases(11.1l%), (+) in 7 (38.9%) and (++) in 9(50.0%), respectively. MVD was 6.89?.54 in finasteride treated group and 9.17?.63 in the nontreatment group, with significant difference statistically .Conclusions In the progress of benign prostatic hyperplasia, the expression of VEGF and angiogenesis are significant developed . VEGF plays a essential role in pathogenesis and development of BPH, VEGF is positively correlate to MVD. Finasteride can downregulate the synthesis and expression of VEGF, which inhibits the angiogenesis in BPH.
