Objective:To organize,induction of chronic obstructive pulmonary disease(COPD) in the etiology,pathogenesis and classification of TCM and Western medicine research progress; and find the correlation between clinical data on the analysis of chronic obstructive pulmonary disease syndromes with clinical parameters (blood count,ESR,liver and kidney function,lung function, blood gas analysis),for TCM syndrome type and therapy.Methods:100 patients diagnosed as COPD,according to their clinical symptoms,signs and tongue,pulse of the syndrome are divided into syndrome of turbid phlegm obstructing lung, syndrome of phlegm-heat obstructing lung,syndrome of phlegm and blood stasis obstructing lung,lung-kidney qi deficiency syndrome.Blood count,ESR,liver and kidney function,lung function,blood gas analysis and other clinically relevant indicators are recorded.Then obtain the correlation between different syndromes of COPD and its clinical parameters by statistics.Results:Male smokers are significantly higher than females(P<0.05).As the disease advances,the average age of patients and the average duration are growing(P<0.05);the body mass index of lung and kidney Qi deficiency syndrome decreased significantly(P<0.05);white blood cell count and the percentage of neutrophils about syndrome of phlegm-heat obstructing lung are significantly increased,with significant difference(P<0.05);Hematocrit of syndrome of phlegm and blood stasis obstructing lung is significantly increased,which is significantly different from the other groups(P<0.05);Serum albumin and Prealbumin of the lung-kidney qi deficiency syndrome are significantly decreased,but its Glucose was significantly increased,which was significantly different from the other groups(P<0.05);The pulmonary function of syndrome of turbid phlegm obstructing lung and syndrome of phlegm-heat obstructing lung are gathered in gradeⅡ,the other two are gathered in gradeⅣ;From syndrome of turbid phlegm obstructing lung,syndrome of phlegm-heat obstructing lung,syndrome of phlegm and blood stasis obstructing lung to lung-kidney qi deficiency syndrome,the patient's level of FEV1%Pred,FEV1/FVC(%),MVV%,DLCO% gradually decreased,and RV/TLC(%) gradually increased.(P<0.05);The PH level of lung-kidney qi deficiency syndrome significantly decrease,and its CO2CP level significantly increase (P<0.01); As the disease advances,the level of PO2 decrease,while the level of PCO2 is gradually increased.Conclusion:COPD is associated with smoking;the different TCM syndrome types have some relevance with blood cell analysis,liver and kidney function,lung function,blood gas analysis:The body mass index, Serum albumin, Prealbumin of the lung-kidney Qi deficiency are significantly decreased,whose Glucose is significantly increased.Hct of the phlegm and blood stasis obstructing lung is significantly increased, WBC,N% and ESR of phlegm-heat obstructing lung are significantly increased,with the progress of the disease,the level of FEV1% Pred, FEV,/FVC(%),MVV%,DLCO%,PO2 are decreased;and the level of RV/TLC(%),PCO2 are gradually increased.The PH level of the lung-kidney qi deficiency syndrome is significantly decreased,while its level of CO2CP is significantly increased,often seen acidosis.All of above provide reference for clinical diagnosis and treatment.
