Sudden deafness is the sudden unexplained sensorineural hearing loss may be associated with tinnitus and vertigo. The incidence of the sudden deafness is an increasing trend in the past decade years. Its pathogenesis is unknown. In recent years, some researchers believe that sudden deafness is a kind of psychosomatic diseases. The research of sudden deafness focuses on correlative study on psychological care and the prognosis of sudden deafness patients in china, but study on the emotional characteristics and coping styles of patients is less. The purpose of the study is:1. To explore the current negative emotional states in sudden deafness patients such as depression, state anxiety and state anger etc.2.To analyze the emotional characteristics (such as trait anxiety, trait anger), emotional expression and personality in sudden deafness patients. Trying to discover whether the incidence of sudden deafness relate to the specific emotional characteristics, personality, coping styles. To explore the relationship between the psychological symptoms and the poor coping styles in the patients.3,To observe the impact of biofeedback therapy to the negative emotions and the prognosis of sudden deafness patients.Research I:The personality, emotional characteristics, coping styles in sudden deafness patientsMethod:We randomly selected 58 hospitalized patients with sudden deafness patients as a sudden deafness group and 40 healthy volunteers as healthy control group in Otolaryngology wall of Qilu Hospital in Shandong Province. They were required to complete the State-Trait Anxiety Inventory (STAI), State-Trait Anger Questionnaire (STAXI-2), self-rating depression scale (SDS), Symptom Checklist (SCL-90), Eysenck Personality Questionnaire (EPQ)and a self general information questionnaire. To compare current emotional characteristics of sudden deafness patients group with healthy control group by independent sample t test. To analyze personality, emotional expression and coping style between sudden deafness patients group and healthy controls by independent sample t test. The relations between psychological symptoms and coping style were checked by Person correlation test. The correlation of state-trait anxiety and coping style was tested by Person correlation analysis.Results:(1) The sudden deafness patients group scored higher than the control group in state anxiety (P<0.01), depression (P<0.05), trait anxiety (P<0.01), anger-in (P<0.05), the state angry was not found to be significantly difference from control group (P>0.05). (2) The sudden deafness group in somatization (P<0.01), forced (P<0.05), interpersonal sensitivity (P<0.05), hostility (P<0.01), terrorist (P<0.05), mental (P<0.01), anxiety (P<0.01), depression (P<0.05) was higher than the control group. Paranoia (P>0.05) had no significant difference. (3) sudden deafness patients group scored higher self-blame than control group (P<0.05). (3) Nine symptom factors of SCL-90 was positively related to immature coping style (self-blame, fantasy, avoidance et.), depression and interpersonal sensitivity was negatively correlated to solving-help coping style.ResearchⅡ:EMG biofeedback interventions on sudden deafness patientsMethods:We knew the sudden deafness patients had higher depression and anxiety than healthy volunteers.The sudden deafness patients were randomly divided into two groups, each group had 26 patients. Control group was given general psychological care, the experimental group was given EMG biofeedback relaxation training in addition to a normal psychological care, every 30 minutes, do it 7 times as a course of treatment. After 7 days the experimental group and control group patients were required to fill STAI, SDS. Results:(1) The subject's the minimum value and baseline of the last EMG were lower than the first EMG after seven days (P<0.05). (2) The level of anxiety of the experimental group was lower than control group after seven times intervention (P<0.05). The level of SDS of the experimental group were no significantly difference before and after intervention (P>0.05). (3) The experimental group was better than the control group on the hearing improvement (p<0.01) after 7 interventions.We have reached the conclusions as follows:1,Compared to healthy volunteers, the sudden deafness patients have more psychological symptoms(such as depression, anxiety, somatization, interpersonal sensitivity, etc.).We should help them to reduce stress and negative emotions in clinical treatment and care.2,Sudden deafness patients are susceptible to stress. In emotional expression, they don't like to express their emotion especially negative emotion and suppress their anger to point to the inside. When they deal with stress, more sudden deafness patients take the self-blame coping style. Anger-in and self-blame coping style probably the one of the causes of sudden deafness patient.3,EMG biofeedback relaxation training can help alleviate the negative emotions of sudden deafness patients and improve their hearing. EMG biofeedback relaxation training is a kind of no damage, no pain, simple and effective therapy technology. As adjuvant therapy we could use EMG biofeedback technology to improve the prognosis of sudden deafness and reduce the risk of recurrence.
