Objective:To discuss the difference of ERCC1 detection by different NSCLC tumor sample category and the relationship between ERCC1 expression and resistence to cisplatin,providing important theoretical evidence to guide sample category choosing for ERCC1 detection and cisplatin's individual chemotherapy in NSCLC field.Methods:1.60 fresh operative NSCLC tumor samples were collected which is divide to 3 parts respectively be tested the chemical sensitivity in vitro and ERCC1 expression.2. One part fresh tumor sample was tested the sensitivity to cisplatin by Cell-Counting kit-8 assay immediately.3. Fresh frozen section and paraffin-embedded section were made from the another two parts tumor samples,and ERCC1 expression of the two kind section samples was determined through immunohistochemical analysis (MaxvisionTM method).4. Analyze the difference of ERCC1 detection by different NSCLC tumor sample category,and the relationship between the different ERCC1 protein expression of two section samples and sensitivity of NSCLC to cisplatin in vitro. Statistics method involved Chi-square test and Fisher's Exact Test.Results:1.There were 27(45%) cases sensitive to cisplatin in the 60 non-small cell lung carcinomas specimen, and still another 33(55%) resistant cases.2. In the 60 cases NSCLC specimen,we use fresh frozen section and paraffin-embedded section to dectec ERCC1 expression, the positive expression was respectively 18 cases and 32 cases (30.0% vs 53.3%).The positive expression rate dectected by paraffin-embedded section was obviously higher than dectected by fresh frozen section (P=0.004),and there is a better positive strength and clear background under the microscope by paraffin-embedded section.3. There were 9(28.1%) cases sensitive to cisplatin in vitro in the 32 cases of ERCC1-positive group, and 18(64.3%) sensitive cases in the 28 cases of ERCC1-negative group dectected by paraffin-embedded section. The rate of sensitivity to cisplatin in ERCC1-negative group was significantly higher than the postive group (P <0.05); there is no statistics difference between the rate of sensitive to cisplatin in vitro in ERCCl-positive group and the rate in ERCC1-negative groupwhen dectected by fresh frozen section(P>0.05).4. There were 18 cases sensitive to cisplatin in vitro in 28 ERCCl-negative cases dectected by paraffin-embedded sections, the coincidence was 64.3% between ERCC1-negative and sensitive to cisplatin; and there were 21 cases sensitive to cisplatin in vitro in 42 ERCC1-negative cases dectected by fresh frozen sections, the coincidence was 50.0% between ERCC1-negative and sensitive to cisplatin,and there is no statistics difference(P=0.327).5. There were 23 cases resistent to cisplatin in vitro in 32 ERCC1-positive cases dectected by paraffin-embedded sections, the coincidence was 71.9% between ERCC1-positive and resistent to cisplatin; and there were 12 cases resistent to cisplatin in vitro in 18 ERCC1-positive cases dectected by fresh frozen sections, the coincidence was 66.7% between ERCC1-positive and resistent to cisplatin,and there is no statistics difference(P=0.754).6. There were 11 cases resistant to cisplatin in vitro in 15 ERCC1-positive cases both dectected by paraffin-embedded and fresh frozen sections,the coincidence was 73.3% between ERCC1-positive and resistant to cisplatin;and there were 16 cases sensitive to cisplatin in vitro in 25 ERCC1-negative cases both dectected by paraffin-embedded and fresh frozen sections,the coincidence between ERCC1-negative and sensitive to cisplatin, and the is statistics difference(P=0.048).7. There were 12 cases resistant to cisplatin in vitro in 17 ERCC1-positive cases dectected by paraffin-embedded sections but ERCCl-negative dectected by fresh frozen section,the coincidence was 70.6% between ERCC1-positive and resistant to cisplatin;and there were 1 case resistant to cisplatin in vitro in 3 ERCC1-negative cases dectected by paraffin-embedded sections but ERCC1-positive dectected by fresh frozen sections, the coincidence was 33.3% between ERCC1-positive and resistant to cisplatin and there is no statistics difference(P=0.270). Conclusions:1.The chemosensitivity in vitro result show that more than 50% NSCLC may resistant to cisplatin2. Paraffin-embedded section samples have a better positive stength and clear background under the microscope,which can act as the standard method to detect the postoperative ERCC1 exepression of NSCLC.3. The ERCC1-negative dectected by paraffin-embedded sections has a higher of rate of sensitivity to cisplatin in vitro than ERCC1-positive,but no obviously difference when dectected by fresh frozen sections,.4.The conformity rate between ERCC1-ngetive and sensitive to cisplatin in vitro,and the conformity rate between ERCC1-positve and resistant to cisplatin when dectected by paraffin-embedded sections are higher than the rate when dectected by fresh frozen sections,but no statistics difference.5. ERCC1-positive expression is related to NSCLC's resistance to cisplatin, ERCC1 is expected to be a prognostic factor of non-small cell lung cancer's chemotherapy with cisplatin.
