Objective:To investigate the effects of remifentanil pretreatment on the heart and lungs against hemorrhagic shock injury in rabbits.Methods: Forty healthy rabbits weighing 2.0~2.5kg were randomly divided into 5 groups (n=8 each): control group NS (group C n=8), Low-dose remifentanil group (R1 group, n = 16; According to shock time R1 group was divided into one-hour group RO1 n=8; two-hour group RT1 n=8,), High-dose remifentanil group (R2 group, n = 16, According to shock time R2 group was divided into one-hour group RO2 n=8; two-hour group RT2 n=8). R1 group remifentanil 0.66μg/kg/min was pumped 15 minutes before blooding, R2 group remifentanil 1.32μg/kg/min was pumped 15 minutes before blooding, group C NS continued to pump into same volume of normal saline. The model of acute hemorrhagic shock was established by modified Wigger's methods. Arterial blood samples (0.5ml)were taken at before bloodletting, 10 min, 40min, 70 min , 100 min and 130min, after bloodletting for measuring mean arterial blood pressure(MAP),heart rate(HR) , performing serum troponin I (CTnI) and detecting arterial bolld gas analysis, and the animals were sacrificed. The hearts and lungs were immediately removed for histological examination with light microscope, electron microscope and determination of wet/dry lung weight (W/D) ratio and MDA content, determination of SOD and NO activity.Results: Heart rate and serum cTnI concentrations were significantly lower in group R1 and R2 than in group C and serum cTnI concentration lower in group RT2 than in group RT1 at T5 and T6. Compared with C control group (C), R1 and R2 significantly decreased in W/D ratio, MDA content and increased SOD activity. Compared with group R1, R2 significantly decreased in W/D ratio, MDA content and increased SOD activity. Conclusions: Remifentanil pretreatment with has protective effect against hemorrhagic shock induced heart and lung injury.
