The Detection Of The Chance Of Lymphocyte Subsets In Sentinel Lymph Node

Objective:To detect lymphocyte subsets in Sentinel lymph node with and without cancer cells metastasis in breast cancer .Methods:From January 2010 to December 2010 , 36 patients with breast lumps were selected, who were diagnosed as infiltrating ductile breast cancer , infiltrating lobular breast cancer or Infiltrating ductal - lobular mixed breast cancer after Lumpectomy. Two milliliter methylene blue(1% ) were injected in the subcutaneous upon tumor around areola, to identify the sentinel lymph nodes. All the patients were accepted modified radical mastectomy .One or two sentinel lymph node and 1～2 non-sentinel lymph node were taken out to study for immune function in each patient. Half of each lymph node specimen was sent to have a pathologic examination. Another half was prepared in unicellular float liquid and flow cytometry was used to to detect lymphocyte subsets, and the ratio of CD₃⁺T,CD₄⁺T,CD₈⁺T,CD4+/ CD8+,CD₁₆⁺NK,CD₅₆⁺NK was compared with each other. Result: Fifty-nine SLN were successfully identified in 36 patients, the ratio of success was 97.2%(35/36),among which 31 SLN were found to have metastases. Sixty-four NSLN near the SLN were taken out in 36 patients, among which 26 NSLN were found to have metastases. The results were founded that the count of CD₃⁺T,CD₄⁺T,CD₈⁺T,CD₁₆⁺NK and CD₅₆⁺NK in SLN without metastasis were 63.25±12.14,53.46±11.24,13.18±11.58,1.25±1.15 and 25.25±1.15,respectively. The main lymphocyte subsets of T cells was CD₄⁺T. The count of CD₃⁺T,CD₄⁺T,CD₈⁺T,CD₁₆⁺NK and CD₅₆⁺NK in SLN with metastasis were 69.31±12.54,36.98±9.54,30.64±12.47,2.64±1.96 and 34.64±1.96,respectively. The ratio of CD₄⁺T decreased, but the ratio of CD₈⁺T,CD₁₆⁺NK,CD₅₆⁺NK rised. The count of CD₃⁺T,CD₄⁺T,CD₈⁺T,CD₁₆⁺NK and CD₅₆⁺NK in NSLN without metastasis were 62.79±13.05,51.59±12.27,11.32±12.13,1.37±1.42 and 24.37±1.42. The count of CD₃⁺T,CD₄⁺T,CD₈⁺T,CD₁₆⁺NK and CD₅₆⁺NK in NSLN with metastasis were 65.87±14.63,39.87±11.31,23.59±11.35,2.42±1.78 and 32.42±1.78. The ratio of CD₄⁺T decreased, but the ratio of CD₈⁺T,CD₁₆⁺NK,CD₅₆⁺NK rised. By SPSS statistical analysis it was founded that the difference of percentage of CD₄⁺T,CD₈⁺T,CD₁₆⁺NK,CD₅₆⁺NK in SLN with metastasis and without metastasis had statistical significance ( P < 0.05) , but the difference of percentage of CD₃⁺T had no statistical significance ( P >0.05).Conclusions:The percentage of lymphocytes CD₄⁺T,CD₈⁺T,CD₁₆⁺NK,CD₅₆⁺NK is normal in SLN without metastasis , and the immune function is normal. While the percentage of lymphocytes CD₄⁺T,CD₈⁺T,CD₁₆⁺NK,CD₅₆⁺NK changed and it`s immune entironment is transformation, and the immune function is bated in SLN with metastasis.
