Objective: Our purpose was to determine whether the residual amniotic fluid index(AFI) of premature rupture of membrance(PROM) is associated with pregnant outcome.Methods: 128 pregnant women with PROM in our hospital from September 2009 to September 2010 were performed and analyzed, we divided into two groups according to the residual AFI: AFI>5cm(n=86), AFI≤5cm(n=42). The comparision between the two groups in delivery, intrauterine infection, neonatal morbidity and perinatal outcome. Results: We found the rate of uterine-incision delivery in the groups of AFI≤5cm(83.3%) was significantly higher than the group of AFI>5cm(17.4%); The rate of natural labor in the group of AFI>5cm(72.1%) was significantly higher than the group of AFI≤5cm(7.1%). The delivery has difference between the two groups(P<0.05).The incidences of amniotic fluidⅢ°stained(57.1%), intrauterine infection(30.9%) and apnoea neonatorum(14.3%) in the group of AFI≤5cm were significantly higher than the group of AFI>5cm(17.5%, 3.5% and 1.1%). The perinatal outcome has difference between the two groups(P<0.05).Conclusions: A significantly decreased residual AFI after PROM was associated with pregnant outcome.1. When the residual AFI≤5cm after PROM, the influence on the pregnant women manifested in the rate of uterine-incision delivery and the intrauterine infection have increased. The influence on the newborn manifested in the Apgar has decreased, the rate of fetal distress and neonatal morbidity have increased.2. Uterine-incision delivery is a good delivery way in the residual AFI≤5cm after PROM. So, when the residual AFI≤5cm after PROM on the pregnant women we should broaden the indication of the uterine-incision delivery. 3. Estimate to the residual amniotic fluid index(AFI) of premature rupture of membrance(PROM) is very important in the perinatal stage.
