| Objective:To evaluate the clinical study on rapid diagnosis of premature rupture of membranes(PROM)by amniotic fluid monitoring pad(pad method). Methods:We enrolled 90 late pregnant women who received treatment in Department of Obstetrics,Jiangxi Provincial People’s Hospital during June to December 2016 and divided them into group A(confirmed PROM,n=30),group B(suspected PROM,n=30)and group C(intact fetal membranes,n=30)based on the clinical manifestations and results of speculum examination.Pad method, amniotic fluid crystallization test(crystallization test),and pH determination using pH paper(pH method),and human Insulin-like growth factor binding protein-1 ELISA kit(IGFBP-1method)were all used to diagnose PROM in all cases. The sensitivity, specificity,Youden index, positive and negative predictive values of the pad comparatively. Results:Concretely, the sensitivity, specificity,Youden index, positive and negative predictive values of the pad method(88.33%、90.00%、78.33%、94.64%、79.41%)were all higher compared with both the crystallization test(80.00%、73.33%、53.33%、85.71%、64.71%)and pH method(80.00%、70.00%、50.00%、84.21%、63.64%),the differences were statistically significant(P<0.05).But,There was no significant difference(P 〉 0.05)with the IGFBP-1 method(91.67%,93.33%,85.00%,96.49%,84.85%).Conclusions:The use of the pad method for the diagnosis of premature rupture of membranesis superior to the crystallization method and the PH method, and is similar to the IGFBP-1 method.Amniotic fluid monitoring pads are similar to pads used by women during menstruation and can be used in outpatients and homes. They have the advantages of easy interpretation, easy operation, good specificity, high sensitivity, low price, and non-invasiveness.The pad method has certain clinical feasibility in rapid diagnosis of premature rupture of membranes and can play an important role in pregnancy management. |
