To Compare With The Therapeutic Effect Of Hemiarthroplasty And Cannulated Screw Internal Fixation For Basal Type Femoral Neck Fracture In Elderly Patients

Objective: Analysis and comparison of the therapeutic effect of the treatment of basal type femoral neck fracture in elderly patients with hemiarthroplasty and cannulated screw internal fixation,to approach the clinical significance of the hemiarthroplasty for elderly patients with basal type femoral neck fracture.Method: A retrospective study was carried out in 28 cases of elderly patients with basal type femoral neck fractures treated from our hospital March 2009 to October 2010,11 cases of whom are male,17 cases of whom female.All the patients aged from 66 to 98 years old and the average age was 76.13 cases is left femoral neck fracture,15 cases is right.Among them, 6 cases with diabetes,11 cases with high blood pressure and heart disease,2 cases with COPD,1 case with cerebrovascular disease,1 case with liver and kidney dysfunction.8 cases with two and two more medical problems.28 cases of patients with femoral neck fractures who were treated with hemiarthroplasty or cannulated screw internal fixation.There are 16 cases in Group hemiarthroplasty. According to allgower classification standard Garden on points type:2 cases for typeⅡ,7 cases for typeⅢ,7 cases for typeⅣ.There are 12 cases in Group cannulated screw internal fixation. According to allgower classification standard Garden on points type:4 cases for typeⅡI,6 cases for typeⅢ,2 cases for type IV.Operation time choose 2 ~ 14 days after injury.Recorded the time of the operations,blood lost, postoperative bed time,postoperative complications and postoperative hip function (Harris score)in both groups,to compare by statistical analysis.Results: The operation time of Group hemiarthroplasty was 82.34±21.15 minutes on average, the operation time of Group cannulated screw internal fixation was 75.58±28.54 minutes on average, there was no significant difference (P>0.05).blood loss in operation of Group hemiarthroplasty was 198.78±369.24ml,Group cannulated screw internal fixation was 95.84±193.57ml,it had difference between two groups of patients(P<0.05).Postoperative bed time of Group hemiarthroplasty was10.43±3.82 days,Group cannulated screw internal fixation was 90.45±25.86 days, there was significant difference (P<0.01). 6.25% of Group hemiarthroplasty had postoperative complications,25.00% of Group cannulated screw internal fixation had postoperative complications, postoperative complications have difference between two groups of patients (P<0.05). According to Harris score to compare the function of hip,dykes of hip function of Group hemiarthroplasty was 93.75%,Group cannulated screw internal fixation was75.00%,The function of hip have difference between two groups of patients (P﹤0.01).Conclusion: The treatment with Hemiarthroplasty for elderly patients with basal type femoral neck fracture has significant effects. Hemiarthroplasty for elderly patients with basal type femoral neck fractures can shorten recovery time and decrease complications. Hip function recovery is quick and good,improve the quality of life patients.
