Objective:The study is to analyze the expressions of B7-H4 and hk6 in epithelial ovarian benign tumors, borderline tumor and expression in tumor tissue and serum,,and to explore there role in the development and invasion of epithelial ovarian tumor. And combined with serum CA125, would provide an experimental basis for diagnosis and prognosis of ovarian epithelial tumor.Methods:1 Immunohistochemistry SP was used to detect the expressions of B7-H4,hK6 protein in 90 cases of ovarian epithelial tumor tissue,among this series,25 cases were epithelial ovarian benign tumor,15 cases were borderline tumor and 50 cases were malignant tumor. Including serous cystadenocarcinoma (n=32), non-serous cystadenocarcinoma (n=18).The positive expression rate of B7-H4 and hK6 protein in epithelial ovarian benign tumors, borderline tumors and tumor tissues was compared,and the relationship between expression of B7-H4 and hk6 protein and different clinical pathological parameters in epithelial ovarian cancer tissue was analyzed.2 The double antibody sandwich Enzyme-linked Immunosorbent Assaytechnique (ELISA) was applied to detect serum B7-H4 and hK6 concentration in patients with epithelial ovarian tumor. The levels of them in ovarian benign tumor,borderline tumors and malignant tumor were compared, as well as the degree of sensitivity and specificity for diagnosis of ovarian malignant tumor.3 Statistical analysis:Count data was compared with the chi—square test or Fisher'S exact probability method. Measurement data was expressed as mean±standard deviation,the comparison of multiple mean with t test,and the two-two comparison among the means was done by single factor variance analysis. Setting P<0.05 to have statistically significant.Results:1 In epithelial ovarian benign tumor, positive expression rate of B7-H4 protein was 16%(4/25).In borderline tumor and ovarian carcinoma was 33.3%(5/15),78%(39/50), respectively.The positive expression rate of ovarian cancer group was higher than the previous two groups (p<0.05). While the former two groups,no statistically significant difference (P>0.05). In serous cystadenocarcinoma, the positive expression rate of B7-H4 protein was 93.8%(30/32) while 50%(9/18) in non serous cystadenocarcinoma. The positive expression was significantly lower in ovarian carcinoma patients with stageⅠ,Ⅱ(52.6%)compared with the advanced stageⅢ(93.5%)(P<0.05);the positive rate in well differentiation group33.3%(3/9)was lower than that in moderately and poorly differentiation group 87.8%(36/41) (P<0.05).The expression of B7-H4 was higher in node metastasis95.2%(20/21)than no-metastasis65.5%(19/29)(p<0.05).2 In epithelial ovarian benign tumor, positive expression rate of hK6 protein was 8%(2/25),while 46.7%(7/15)and 60%(30/50) in borderline tumor and ovarian carcinoma respectively.The positive expression rate of benign tumor was lower than those of the laster two groups(P<0.05).while the laster two groups,no statistically significant difference (P>0.05). In serous cystadenocarcinoma,the positive expression rate of hK6 protein was 65.6%(21/32) while 50%(9/18) in non serous cystadenocarcinoma. In ovarian carcinoma patients, the positive expression was lower with stageⅠ,Ⅱ36.8%(7/19)compared with the stageⅢ74.2%(23/31) (P<0.05);the positive rate in moderately and poorly differentiation group 70.7%(29/41) was higher than that in well differentiation group(11.1%) (P<0.05).The expression of hK6 was significantly higher in node metastasis85.7% than no-metastasis41.4%(12/29) (p<0.05).3 In epithelial benign ovarian tumor,borderline ovarian tumor and ovarian carcinoma,the expression level of serum B7-H4 was 0.714±0.049,0.746±0.046, 0.803±0.067(ng/ml),respectively;hk6 was 3.72±0.438,4.43±0.214,4.5±0.474(μg/l), The expression levels of serum B7-H4,hK6 were difference among three groups(P<0.05).B7-H4 protein concentration in ovarian carcinoma was higher than those in the former groups,while no statistically significant difference (P>0.05) in the former two groups.Hk6 protein concentration in benign ovarian tumor was lower than those in the last groups(P<0.05), while no statistically significant difference (P>0.05) in the last two groups.4 The sensitivity and specificity of serum B7-H4 and hk6 in diagnosis of ovarian epithelial tumor was 70%,60%,66%;75%,67.5%,62.5%,respectively. Three combined detection sensitivity for 60%.Conclusions:1 The elevated expression of B7-H4,hk6 in serum and tissue of patients with ovarian epithelial tumor.The changes of their expressions associated with clinical pathology parameter.B7-H4,hk6 could be used to help determine prognosis in patients with epithelial ovarian cancer.2 B7-H4, hk6 may be involved in the occurrence and development of process of epithelial ovarian cancer, detection of serum B7-H4, hk6 CA125 joint may occur as a guide of epithelial ovarian cancer diagnosis of indicator.
