ObjectiveTo explore the clinical value of AFP-L3as a biomarker in diagnosis of hepatocelluar-carcinama (HCC) in different level of AFP.MethodsSrum samples and clinical characteristic of97patients were collected. According to the pathological biopsy and the iconography, all patents were devided three groups, i.e. primary hepatocellular carcinoma group(HCC)(n=37), liver cirrhosis(LC)(n=30) and chronic hepatitis(CH)(n=30). AFP-L3was isolated by microcentrifugalcolumn method, AFP was detected by chemiluminescent immunoassay; then the proportion of AFP-L3levels were calculated(AFP-L3/AFP).ResultThe level of AFP and AFP-L3%were abnormally higher in HCC group than LC or CH group(P<0.05). The corresponding area under the ROC curve were0.686and0.825. both of them reached significant difference(P<0.05); between the level of20-400ng/ml. there was no significant difference for AFP among the groups(P=0.774). but significant difference for(P=0.000). The corresponding area under the ROC curve were0.557(P=0.429) and0.807(P=0.000). Correlation analysis revealled that there was no correlation between AFP-L3%and AFP(r=0.135, P=0.186).ConclusionCombined imaging tests,AFP-L3can be used as an independent factor for HCC diagn-osis. AFP-L3is a better biomarker in early warning of hepatocelluar-ca-rcinama (HCC) in patients with mild to median elevated AFP level than AFP.
