ObjectiveTo establish rat models of inflammatory fallopian tube by injecting Chlamydia trachomatis and Ureaplasma urealyticum through the cervical of rats. To observe the changes of pathology and the micro structure of the fallopian tube, also the influence on the level of transforming growth factor-betal (TGF-β1),tumor necrosis factor-alpha(TNF-α) on fallopian tube epithelium by intravaginal inoculation of Salvia miltiorrhiza injection. To observe mechanism of action of Salvia miltiorrhiza injection on salpingitis infertility patients after interventional recanalization.MethodsSixty three female SD rats, ranging from8-10weeks of age, divided into Blank group three and experimental group sixty. Experimental group were inoculated progesterone by intramuscular injection daily, then divided into Ct group and thirty and Uu thirty. Rats in Ct group were inoculated50ul×107of inclusion-forming units of Chlamydia trachomatis E serovar on left uterine horn while rats in Uu group were inoculated50ul×107of inclusion-forming units a week later, then inoculated progesterone by intramuscular injection every other day,4times totally. After that, the two group were randomly divided into four subgroups:medium does of Salvia miltiorrhiza(Sm) group, high does of Salvia miltiorrhiza(Sm) group, Western medicine(Wm) group and Normal saline(Ns) group. Rats in four groups were inoculated the corresponding dose of medicine once every three days,3times totally. After that, observe the changes of pathology and the micro structure of the fallopian tube, the influence on the level of transforming growth factor-betal (TGF-β1),tumor necrosis factor-alpha(TNF-α) on fallopian tube epithelium by intravaginal inoculation of Salvia miltiorrhiza injection as well.The clinical research:Collect patients which were diagnosed as salpingitis infertility and accepted interventional recanalization successfully, randomly divided into SM group and Wm group. Then compare the effects after the treatments. We try to explain the mechanism of Chinese medicines for promoting blood circulation, such as Salvia miltiorrhiza, through animal experiments and clinical research.ResultVisual observation:Compared the Ns groups and the other groups, we found that uterus of rats which in the Ns groups turned white, stiff, some of them were enlarged and filled with inflammatory liquid.It was relieved after the treatments. However, the number of enlarged uterus in Ct groups was more than that in Uu groups. Some of the uterus had inflammatory nodules in Ct groups, which was more than that in Uu groups, and there was none in the blank group. Inhensions between connective tissue roud the uterus and fallopian tube were reltively serious in Wm groups and Ns groups, least in medium dose of Sm groups.Light microscope:①Compare the pathological changes of the rats' fallopian tube:Fallopian tubes in Ns groups obviously turned white, stiff, some of them were enlarged, and the mucosa plica lodged, fell off or even disappeared. Mucosa plica in proximal tubal side adjoined together, making the tube stenosis. There was significant difference between Ns groups and the normal group (P<0.05). These symptoms relieved in the Sm groups.②Compared the number of neutrophils in bilateral uterine wall of the experimental group at high magnification:There was significant difference between the Sm groups and the Ns groups and also between the Wm groups and Ns groups(P<0.05). However, there was no statistical difference between Ct groups and Uu groups (P>0.05).③Compared the level of TNF-α and TGF-β1in the fallopian tube:There was statistical difference between the experimental groups and the Ns groups (P<0.05), the level of TNF-α and TGF-β1lwere higher in the experimental groups. In the Sm groups, Wm groups and Ns groups, the level was highest in the Ns groups (P<0.05). However, there was no significant difference between the medium does of Sm groups and high does of Sm groups(P>0.05), so was it between the Ct groups and the Uu groups.Scanning microscope:There are ciliated cells and non-ciliated cells on the epithelial of the normal fallopian tube, they row orderly and the cell membrane is complete, the cilia are smooth and row orderly. The ciliated cells in Ns groups rowed inorderly, the border was not clear, the cell membrane was incomplete, the cilia fell off or even disappeared, which were worst than that in the other groups. There were white blood cells lied on the surface of the epithelial of the fallopian tube, however none Ct or Uu were found.Clinic research:Compared with Western medicine, Chinese medicine can promote blood circulation, such as Salvia miltiorrhiza. It can improve intrauterine pregnancy rate after the treatment of Interventional Recanalization.ConclusionsThe uterus and fallopian tubes of the rats were inflammatory after infected with Chlamydia trachomatis E serovar and Ureaplasma urealyticum, which enlarged, or filled with inflammatory liquid. And it had higher the level of TNF-α,TGF-β1. Therefore, we suppose to persuade the infertility patients to have a detection of Uu before pregnant, so as to give them treatments as soon as possible if the result come out positive.Infect rats with Chlamydia trachomatis E serovar and Ureaplasma urealyticum in the uterus can establish models of inflammatory fallopian tubes, like the way human beings infected.Salvia miltiorrhiza injection, inoculated intravaginally, can reduce the secretion of inflammatory cells and suppress the inflammation. We suppose that the reason may probably be it can reduce the level of TNF-α,TGF-β1.Salvia miltiorrhiza injection can relief the inflammation and fiber hyperplasia, keeping the tubal patency and improving pregnancy rate for salpingitis infertility patients after interventional recanalization.
