Objective To evaluate the alteration of left ventricular twist in patients with coronary heart disease (CHD) using velocity vector imaging (VVI), and further to explore its clinical value.Methods 48 patients with CHD and 30 control subjects were enrolled. Patients with CHD were divided into 2 groups according to results of coronary angiography:ischemia group (n=20), MI group (n=28). Two-dimensional images of the left ventricular short-axis views at basal and apical level were recorded. Left ventricular twist indexes were obtained using VVI software.Results Peak twist velocity (PtwV) was significantly reduced in ischemia group when compared with control group (P<0.05). Peak apical rotation (PAr), peak twist (Ptw) and PtwV was significantly reduced in MI group when compared with control group (P<0.05), while maximal temporal difference of time to peak twist (Tw-diff) was significantly increased (P<0.05). Peak twist velocity (PtwV) was significantly reduced in MI group when compared with ischemia group (P<0.05). Peak twist velocity (PtwV) was significantly reduced in MI group when compared with ischemia group (P<0.05). There was a significant step-down change of PtwV among control group, ischemia group and MI group. Significant correlation was found between left ventricular ejection fraction (LVEF) and Ptw, PtwV. (r=0.71,P<0.05; r=0.84, P<0.05).Conclusions Left ventricular twist is injured in patients with CHD. Left ventricular twist indexes especially PtwV can accurately evaluate the alteration of left ventricular twist in patients with CHD. VVI can accurately evaluate the alteration of left ventricular twist in patients with CHD.
