Objective To investigate the diagnostic value and clinical significance of3.0T susceptibility weighted imaging (SWI) in the bleeding lesions of the brain common diseases.Methods A total of130cases from July2011to January2012with craniocerebral diseases proved by pathology, radiology and relevant inspection included30traumatic brain injury,35cerebral infarctions,30hemorrhage,35rain tumors (7gliomas,10meningiomas,6cavernous angiomas,8pituitary adenomas and4metastatic tumors. They underwent routine MRI(T1WI, T2WI and T2FLAIR) and SWI. Of them,70underwent DWI,35underwent contrast enhancement scan,41underwent MRA and6underwent MRV. SWI images were analyzed to evaluate the diagnostic values of SWI in the bleeding lesions of the brain common diseases..Results1. In patients with traumatic brain injury combined with routine MRI sequence and SWI total of356lesions were found,Which SWI detected lesions in334, a sensitivity of93.8%; T1WI, T2WI detected lesions in141, a sensitivity of39.6%;FLAIR detected lesions in193,a sensitivity of54.2%. The detection of SWI is significant higher than detecting rate of routine MRI.2. SWI and conventional MR sequences were performed in35patients with acute cerebral infarction. Hemorrhagic cerebral infarctions were found in20cases,in which20cases showed clearly in SWI,the display rate of100%;7cases showed clearly in conventional MRI sequences,the display rate of35%.All cases showed the scope of bleeding greater in SWI than conventional sequence. 3. In30cases of hypertensive intracerebral hemorrhage, the number of cerebral microbleeds were124detected by T2-weighted image,273detected by T2FLAIR and1243detected by susceptibility weighted imaging, respectively. There was significant difference between sequences for the detection of cerebral microbleeds.4.41tumors were found in35patients. In the evaluation of the bleeding lesions, the positive rate of SWI (92.1%) was significantly higher than that of conventional MRI (35.6%); among the35tumors,23calcification were found; SWI could show necrosis and substantial part of tumor similiar to conventional sequences; SWI displayed vasculature of tumors in40cases, which were not found by conventional sequences, among6cases,tumor-draining blood vessels were found in2cases by MRA;there was close correlation between pathological results and SWI in depicting internal architecture of lesions.Conclusion SWI is very sensitive to small veins and iron deposition of brain. It can show venous system of normal brain clearly and monitor the changes of distribution and concentration of normal brain iron deposition.
