PART I A preliminary study of the prenatal systematic screening in fetal congenital heart diseaseObjective:To explore the significance of the discovery of fetal congenital heart disease by prenatal systematic screening four chamber view of the cross-section of the heart, left ventricular outflow tract, right ventricular outflow tract and three vessels trachea view.Methods:Among the8632cases of prenatal screening of pregnant women in November2010to March2012, we have used four chamber view, left ventricular outflow tract, right ventricular outflow tract,and three vessels trachea view to screen the heart systematically, for all the positive and suspected positive cases passing into the next process for detailed echocardiography examination.Result:Of the8632cases, fetal detection of fetal cardiovascular positive77cases and suspected positive17cases and a total of94cases, we found cardiac abnormalities in77cases,73.9%sensitivity,17cases of suspected abnormal, complex fetal cardiac malform ations36cases, the sensitivity and specificity respectively88.1%and90.3%.Conclusion:The4prenatal systematic screening standard section in the diagnosis of fetal cardiovascular abnormalities have high sensitivity and specificity, and lay the foundation for further fetal cardiovascular ultrasound diagnosis. PART II The application value of fetal heart ultrasound examination in fetal congenital heart diseaseObjective:To explore the implicational value of the standardization of prenatal cardiac ultrasound examination for the crosssection of the heart four chamber view at the surface, left ventricular outflow tract section, right ventricular outflow tract, the three vessels trachea view, aortic arch section, pulmonary artery catheter bow section, vein atrial connection section and so on, nonstandard section and continuous dynamic scanning were conbined when necessary in fetal congenital heart disease.Methods:Select the hospital antenatal system screening in November2010to March2012found94cases of abnormal and suspicious abnormal, outside the hospital referral over the consultation937cases, a total of1031cases of fetal prenatal cardiac examination standardized section scanning the fetal heart.Result:Fetal cardiac examination normal scanning method detected147cases of fetal congenital heart disease, sensitivity94.3%, and specificity98.5%. When screening four chamber view, the section of left ventricular outflow tract and right ventricular outflow tract section, and the three vessels trachea view section, of the four section scanning,5cases were misdiagnosed and9cases were missed diagnosis, In the preliminary suspicious abnormal cases,12cases were confirmed positive cases.Conclusion:Fetal heart check specification section scanning can find the detailed inform ation and accurate diagnosis of fetal cardiac malformations, and has important clinical value for clinical treatment and prognosis for fetal assessment, It is the first choice of Tech nology for fetal cardiac examination.
