Background:Congenital heart disease is the most common fetal malformation.The incidence of congenital heart disease in live births is about 0.8%,and the incidence in the fetal period is higher,about 4-10%.Congenital heart disease currently accounts for the first birth defects in newborns and is the leading cause of death in the first year of neonatal.Detection of fetal congenital heart disease early in pregnancy is important for perinatal medicine and eugenics.Fetal cardiac ultrasound has been recognized internationally as the preferred method for clinical screening and diagnosis of congenital heart disease because of its safety,efficacy,noninvasive and repeatable.Objective:To establish the relationship curve between fetal heart development and gestational age by studying the normal inner diameter range of fetal heart and large blood vessel at 20-28 weeks of gestation,provide reference for prenatal consultation of congenital heart disease.Methods:2267 pregnant women who underwent routine prenatal screening were selected for color doppler examination of the fetal heart.The fetal heart data at different gestational weeks were collated,analyzed and summarized to explore the relationship between fetal heart development and gestational age,and initially establish the curve of the relationship between fetal heart development and gestational age.Results:As the gestational age continues to increase,the inner diameter of the heart chamber and large blood vessels also gradually increase.The differences in parameters with the same name at different gestational weeks were statistically significant(P < 0.05).Among them,the right atrium,right ventricular diameter and gestational age were most closely related(r=0.730,r=0.739),and the correlation between left atrium and gestational age was slightly worse(r=0.248).The inner diameter of the fetal pulmonary artery is wider than the aorta in the middle pregnancy and the foramen ovale and arterial catheter increase with increasing gestational age.Conclusion:1.Prenatal ultrasound is an important means of screening for congenitalheart disease currently,and prenatal screening is recommended for all pregnant women.2.The anatomical structure and hemodynamic characteristics of the fetal heart are different from those of the adult.The study on the anatomical structure and hemodynamic characteristics of the fetal heart is of great significance for prenatal screening of congenital heart disease.3.By studying the parameters of normal fetal heart and large blood vessels during mid-pregnancy(20-28 weeks),this paper discusses the characteristics of fetal heart development,and preliminarily establishes the relationship between fetal heart development and gestational age,which can provide reference for prenatal ultrasound evaluation of fetal heart development.
