Objective Explore the1.5T MRI DWI sequence LAVA dynamic enhanced sequences in the detection of small hepatocellular carcinoma, correct diagnosis and differential diagnostic value.Method Collected30cases of pathology, DSA, AFP integrated confirmed the MRI image data of the patients with small hepatocellular carcinoma,Underwent conventional spin echo (T2WI T1WI) sequence DWI (b value of0,500s/mm2), the symbol LAVA dynamic contrast-enhanced (arterial phase, portal venous phase and balanced three) scan,(1) measurement of SHCC lesions, surrounding liver tissue ADC values, SI, and SD.Comparison of DWI, T2WI, three different sequences of T1WI lesion detection sensitivity.(2) measurement of the level sweep T2WI, T1WI and enhanced signal intensity (SI) and the random background noise (SD) to calculate the contrast to noise ratio (CNR), contrast enhancement rate (CER)(3) depicts the CHCC and the surrounding liver tissue the time_signal intensity curve.Result Showed low signal in TlWIof SHCC, SI=(5.949±2.031) x102,T2high signal performance of the SI=(6.386±2.116) x102,DWI showed high signal performance, SI=(3.719±1.535)x102, CNRT1=8.238±4.415, CNRT2(4.717±2.614)x10, CNRDw1=21.19±1.331; CNRT1and CNRDW1P=0.000statistically significant, indicating that the DWI sequence with T1sequences compared, easy to detect lesions; ADCSHCC value (1.177±3.573) x10-3, the ADC value of surrounding liver tissue(1.301±3.284) x10-3the two comparison:P=0.000statistically significant, indicating that SHCC restricted diffusion, was the performance of high signal on DWI images, ADC value is smaller than the surrounding liver tissue;(3) SHCC in the arterial phase, portal phase, the balance of contrast enhancement ratio (%)1.099±0.652,0.501+0.219,0.414±0.379,the surrounding liver tissue were0.019±0.050,0.587±0.338,0.669±0.406(1) the SHCC group and the surrounding liver tissue group CER (%) analysis of variance, F values were22.430,10.226,4.146, P value are as follows:0.000,0.003,0.048, treatment groups were statistically significant (2)between any two q test:the portal venous phase and equilibrium phase, the arterial phase and portal venous phase, the arterial phase and equilibrium phase belonging to different fields, p<0.05,The difference was statistically significant.(4) The SHCC time-signal intensity curve of17cases of speed1downhill type, seven cases is rising type, six cases of ramp slow down type liver tissue time-signal intensity curve to rise slowly.Conclusion DWI and T1WI lesion detection sensitivity, the symbol LAVA dynamic contrast-enhanced to better grasp the lesion enhancement time reveal the blood supply of the lesion to determine benign from malignant lesions, combined with coronal imaging, which fully demonstrates the morphologic characteristics of lesions the combination is conducive to the detection of small liver cancer, benign and malignant judgment, increase the rate of correct diagnosis of small hepatocellular carcinoma, but also conducive to the differential diagnosis of perihepatic space-occupying lesions.
