Objective: The study by flow cytometry of ascites to detect the proportion of CD8﹢CD28﹣T cells and CD8~﹢CD28~﹣NRP-1~﹢T cells in benign and malignant ascites toinvestigate the local immune mechanism, and may provide a new technology for theearly diagnosis of malignant ascites.Methods: A total of72samples were collected, including hepatocirrhosis ascites(n=24) tuberculous ascites (n=20) and malignant ascites (n=28). CD8~﹢CD28~﹣Tcellsand CD8~﹢CD28~﹣NRP-1~﹢T cells was detected by flow cytometry.Results:①The percent of CD8~﹢CD28~﹣Tcells in malignant ascites group (24.75±4.09)%washigher than hepatocirrhosis ascites (16.15±1.49)%tuberculous ascites group(13.83±2.38)%,The different was statistically significant (P <0.05).②The level of CD8~﹢CD28~﹣NRP-1﹢T cells in maligant ascites group was(26.03±2.20)%,The level of CD8~﹢CD28~﹣NRP-1~﹢T cells in hepatocirrhosis ascites(19.53±3.49)%,The level of CD8~﹢CD28~﹣NRP-1~﹢T cells in tuberculous ascites group(14.74±1.18)%,Malignant ascites group CD8~﹢CD28~﹣NRP-1~﹢T cells levels weresignificantly higher than that of hepatocirrhosis ascites and tuberculous ascites group.The difference was statistically significant (P<0.05). the sensitivity of CD8﹢CD28﹣NRP-1﹢T cells>(24.75±4.09)%to the diagnosis of the malignant ascites is78.57%,the specificity is95.45%,The diagnostic accuracy was88.89%, the positivepredictive value of82.14%With ascites CD8~﹢CD28~﹣NRP-1﹢T cells as the boundaryvalue.③The percent of CD8~﹢CD28~﹣Tcells and CD8~﹢CD28~﹣NRP-1~﹢T cells was nosignificant difference (P>0.05) in malignant ascites with different patholo gical types.④The percent of CD8~﹢CD28~﹣Tcells level in malignant ascites has a significantly positive correlation with that of CD8~﹢CD28~﹣NRP-1﹢T cells (r=0.926,P <0.01).⑤The CD8~﹢CD28~﹣NRP-1﹢T cells level in malignant ascites local immunemechanism has a significantly positive value.Conclusion: CD8﹢CD28﹣T cells and CD8~﹢CD28~﹣NRP-1﹢T cells was significantly differentbetween malignant ascites hepatocirrhosis ascites and tuberculous ascites and theycould be used to distinguish malignant ascites from hepatocirrhosis and tuberculous.The relative increase in CD8﹢CD28﹣T cells and CD8~﹢CD28~﹣NRP-1﹢T cells may be relatedto the immunosuppression and tumor progression in patients with malignancies.Analysis the level of CD8﹢CD28﹣T cells and CD8~﹢CD28~﹣NRP-1﹢T cells expression couldbe used as a relatively specific index to evaluate the immunity of patients withmalignancy.
