Objective To test the expression of CD28 on the surface of CD8+T-lymphocyte in liver tissue of patients with chronic hepatitis B,observe the relationship between it's expression and the grading of liver inflammation, explore the immune mechanism of Hepatitis B virus (HBV) clearance and liver tissue lesions.To provide a theoretical basis for the clinical treatment of chronic hepatitis B (CHB).Methods Randomly selected 100 patients with chronic hepatitis B, took down their liver tissue by liver biopsy, test CD8+CD28+ and CD8+CD28-T lymphocytes in liver tissue with immunohistochemical double-labeling and the expression of Tumour necrosis factorα(TNF-α),Fas ligand (Fas-L) with immunohistochemical single-labeling.Results⑴CD8+CD28+ cells and CD8+CD28-cells in the liver tissue of patients with chronic hepatitis B increased significantly compared with the control group, the difference was statistically significant.⑵Further study of CHB according to different liver tissue lesion show that in the liver tissue of patients with moderate and severe lesions, CD8+CD28+cells increased significantly compared with the control group. In the liver tissue of patients with mild lesions, CD8+CD28+cells decreased significantly compared with the control group,The difference was statistically significant.⑶In the liver tissue of patients with chronic hepatitis B CD8+CD28-cells reduced gradually with the liver inflammation. Differences between the groups was statistically significant.⑷In the liver tissue of patients with chronic hepatitis B ,the expression of TNF-αand Fas-L significantly increased compared with the control group, and gradually increased with the aggravation of liver inflammation. Differences between the groups was statistically significant.⑸When liver lesions were the same,in patients with normal ALT,the expression of Fas-L was higher than that in patients with abnormal ALT. Differences between the groups was statistically significant. And both the expression of TNF-αwas no significant difference.Conclusion⑴The expression of CD28 on the surface of CD8+T-lymphocyte in liver tissue of patients with chronic hepatitis B is closely related to liver tissue lesion, CD8+CD28+T lymphocytes in liver tissue can be used as immunological markers of liver injury in patients with chronic hepatitis B.⑵CD28 can promote the expression of TNF alpha and Fas– L in CD8 cells, and cause liver lesions in two ways : necrosis and apoptosis. In patients with normal ALT, apoptosis pathway may be the main mode.⑶C D8+CD28-T cells in liver tissue may be the main reason for persistent infection of hepatitis B.
