Study On Gastroesophageal Reflux Disease And Related Diseases By Endoscopy In Putian，Fujian Province

AIM: To evaluate the prevalence of gastroesophageal reflux disease(GERD) and itsrelated diseases by endoscopy in middle and elderly crowd of Putian，Fujian Province，and to investigate the relationship between GERD and Hp.METHODS: With a stratified random sampling method， a upper endoscopicexamination with Lugol’s staining and biopsy was performed in the crowd whose agewere from40to69-year-old in Hanjiang，Putian，Fujian Province.Gastric antrummucosa was taken for rapid urease test to detected Hp..RESULTS:976individuals were taken by upper endoscopy screening（n=976; meanage,52.96±7.59ys;53％female）.35reflux esophagitis(RE)(3.6％,1/3female) werefound.Among them7cases were in40~49years old,17cases in50~59years old，and11cases in60~69years old.But the difference in each age group was not statisticallysignificant(P＞0.05).Mild RE was91.43％，which was far more than severe RE.Barrett’s esophagus (BE) was12.7％（54.75％female).3.23%patients suffered with BEwere also with RE as well.98.39％of BE patients were with short segment BE．Except2patients with esophageal varices, Lugol’s staining was carried out in974subjects.Theresult showed that215individuals had iodine staining abnormal region in esophagus,including176light staining and39unstaining and biopsy were taken at these place.18high-grade intraepithelial neoplasia and9esophageal squamous carcinomas were found.Besides,1cardia cancer（0.10％,1/976）,11distal gastric adenocarcinomas（1.13％,13/976）and2cardia and8distal stomach high-grade intraepithelial neoplasia werefound.The positive rate of Hp.was31.25%,and the difference between male and femalewas not significan（tχ2=0.001，P＞0.05）.In addition,the prevalence of GERD was13.11％in the positive Hp.group,lower than the negative one.However,there was nosignificant difference.CONCLUSIONS: The prevalence of RE is3.6%in middle-aged crowd in Putiancity,Fujian Province and the prevalance of BE (12.7%)is remarkable higher than that of other areas.The infection rate of Hp. was lower than that in general population in China.The endoscopic screening with Lugol’s staining maybe help to improve the esophageallesion detection rate.
