Objective To detect the level of serum1,25-dihydroxy vitamin D3in patients withdiabetic nephropathy, so as to explore its role in the progression of diabeticnephropathy.Methods According to the urinary albumin excretion rates trace(UAER),131patients with type2diabetes were divided into diabetes group (DM group), earlydiabetic nephropathy group (EDN group), clinical diabetic nephropathy group (CDNgroup), and end-stage diabetic nephropathy group (ESDN group),30cases of physicalexamination as healthy controls group(HC group).The level of serum1,25(OH)2D3and25-OH-D3were detected by enzyme-linked immunosorbent assay (ELISA).Results1. The level of serum1,25(OH)2D3(11.95±0.84pg/ml) in DM group wassignificantly lower than in HC group(29.0±10.0pg/ml)(P<0.01), The level of serum1,25(OH)2D3(10.63±0.85pg/ml) in EDN group was lower than in DM group (P<0.05),The level of serum1,25(OH)2D3(8.71±1.17pg/ml) in CDN group was lower than inEDN group (P<0.05), The level of serum1,25(OH)2D3(6.17±1.05pg/ml) in ESDNgroup was significantly lower than in CDN group;2.The level of serum25-OH-D3(13.18±4.46ng/ml) in DM group was lower than in HC group(15.8±6.0ng/ml)(P<0.05), The level of serum25-OH-D3(12.65±3.22ng/ml) in EDN group was lowerthan in DM group, The level of serum25-OH-D3(9.87±4.26ng/ml) in CDN groupwas lower than in EDN group(P<0.05), The level of serum25-OH-D3(9.87±4.26ng/ml) in ESDN group was significantly lower than in CDN group(P<0.01);3.Thelevel of serum1,25(OH)2D3was lower than25-OH-D3in DM and DN group withrenal injury progression.4.The level of serum1,25(OH)2D3was negative correlationwith UAER(r=-0.452,P<0.05), and was positive correlation with GFR in DM and DN group(r=0.390, P<0.05).Conclusion1.1,25(OH)2D3and25-OH-D3are deficiency in DM and DN patients.The deficiency is more and more obvious with renal injury progression.2.With theprogress of the renal injury in DM patients, the level of serum1,25(OH)2D3is lowerthe level of serum25-OH-D3.3.The level of serum1,25(OH)2D3was negativelycorrelated with UAER and was positively correlated with GFR in DM and DNpatients. It may be a sensitive indication for diagnosis of early renal injury andprogression in patients with DN.
