PurposeBy using of DWI (diffusion-weighted imaging) and MRA (magnetic resonanceangiography) to study the image manifestations and pathogenic forms of Infarctioncaused by stenosis and occlutions on the MCA due to unilateral atherosclerosis, anddiscuss its potential nosogenesis, which could provide evidence for individualtreatment in the clinical acute period and for secondary prevention.MethodsTaking131patients as samples whose onset within72hours and by DWIdiagnosis, infarctions with atherosclerosis MCA stenosis and occlusion, diagnosed byMRA, the MCA stenosis (≥50%)or occlusion is without pathological changes on theipslateral internal carotid. By using DWI series, we study on the morphology ofinfarction caused by arterial occlusion in the brain, patients with potential cardiacembolus.Result1. No statistics difference (p>0.05) in each risk factor of MCA stenosis andocclusion.2. In MCA occlusion group, patients whose infarction involves water-shed ismore than that of stenosis group with occlusion67.6%while stenosis18.5%whichhas differences in statistics(P <0.05).3.1MCA main artery stenosis group has higher infarction rate than that ofocclusion group, with61.5%in stenosis group,23.5%in occlusion group, which hasstatistics difference(P <0.05). MCA main artery stenosis group has lower multipleinfarct rate, with stenosis group38.5%, and76.5%in occlusion group which hasstatistics difference (P <0.05).3.2In MCA main artery stenosis group, PAI is majority in single cerebral infarction(24/32,75%); while in occlusion group, PAI is (6/16,37.5%), which hasstatistics difference (P <0.05)3.3For multi infarction, PAI+PI is the majority in MCA main artery stenosisgroup (12/20,60%), while PAI+PI in occlusion group is (5/29,9.6%), which hasstatistics difference(P <0.05).3.4For multi infarction, PAI+PI+BZI is the majority in MCA stenosis group(33/52,63.5%), while PAI+PI+BZI is (2/20,10%) in occlusion group, which hasstatistics difference(P <0.05)4.For single infarction patients, mainly happens in slow progress (72.9%), whilefor multi infarction patients, mainly happens burstly (65.3%), which has statisticsdifference(P <0.05)ConclusionThe image manifestations of Infarction caused by stenosis and occlutions on theMCA is different;The onset form of single infarction and multi infarction isdifferent.They present different infarction pathogenesis, which shall be treatedindividually according to pathogenesis.
