Objectiv: The present study is to Discussion relevant factors about primary livercancer with Portal vein tumor thrombus in different ranges. analysis the effects ofhepatic function change before and after transcatheter hepatic arterialchemoembolization on the primary hepatic carcinoma with different range of Portalvein tumor thrombus.Methods: There are326primary liver cancers who were treated by the FirstAffiliated Hospital of Henan University of Science and Technology in January2009toNovember2011, and the hospital have recorded the complete clinical data for thesecases. This paper analyzes these cases retrospectively. In order to dig out relativefactors on how portal vein tumor thrombus were formed, this paper has made a logisticregression analysis for those relative factors which are from the patients’ clinical dataand imaging manifestation. About the collected cases, some patients don’t have, whilethe others have the consolidation of different range of portal vein tumor thrombus; Forthe patients who have been tread by TACE (Transcatheter Arterial Chemoembolization)treatment, this paper analyzes the Child-Pugh grade change before and after one weekof the operation, and the AST, ALT changes were also collected and analyzed at thesame time.Results:After making multivariate logistic regression analysis on age, sex,pathological type of tumor, center of the tumor location, tumor pseudocapsule, chronicsecond liver infection, cirrhosis, portal hypertension, hepatic arteriovenous fistula,AST, ALT, AFP, albumin, liver function and so on, it appears there are6kinds ofrelative factors, such as younger, pathological type of tumor, cirrhosis, portalhypertension, hepatic arteriovenous fistula and AST, which were entered logisticregression equation. The mentioned6factors are Risk Factors for primaryhepatocellular carcinoma with portal vein tumor thrombus. Among the collected cases, according to different range of portal vein tumor thrombus, the patients were classifiedinto three groups:nowhere vein tumor, thrombi with portal vein I type,tumorthrombi with portal vein II type. For the patients who have been tread by TACEtreatment, this paper exercises case-control study to analyze the liver function Child-Pugh classification. The results shows that after one week of TACE the patients liverfunction Child-Pugh classification was statistically significant among the three groups.Conclusions: Cirrhosis, younger age at onset, hepatic arteriovenous fistula andthe higher level of ASL are risk factors for primary liver cancer with portal vein tumorthrombus. After one week of TACE the patients liver function Child-Pughclassification was statistically significant among the three groups, and type Ⅱ tumorthrombus is more easily compared with the preoperative progress.
