Purpose:Explore the value of ultrasound contrast in the different diagnosis of benign and malignant lymph node diseases.Materials and Methods:Do ultrasound contrast in27cases patients with superficial lymph nodes diseases, whose result is compared with pathological results. And the result are divided into three groups:the benign group, the metastatic lymph node group and lymphoma group. The contrast agent SonoVue of Italian Bracco Company and was prepared to5ml milky microbubble suspension.Every patient was injected2.4ml SonoVue into their elbow vein. All the patients were diagnosed by color ultrasonic diagnostic apparatus Philips IU22, with a L9-3high-frequency linear array transducer. Examination time is3minutes, Then replay the saved image,and observe the enhanced performance of lymph node imaging. Gamma-variate software is used to describe the time-intensity curve of the regions of interest of the lymph node automatically. Ob served indexes include arrive time (AT), time to peak (TTP), peak intensity (PI) and speed parameter (β). Finally, all the lymph nodes need to do pathologic exam,and comprising pathological diagnosis to the result of ultrasound contrast is to get the diagnostic accuracy of ultrasound contrast. To adopt SPSS13.0software package is to analysis the statistical of the test results. Measurement data expressed as(x±s).The differences among groups were calculated by one-way anova and comparison between groups was made by LSD-t test.p<0.05for the difference was statistically significant.Results:15cases lymph nodes was benign in27cases superficial lymph nodes determined by the pathological diagnosis,2cases were metastatic tumors,10cases were lymphoma. There are14cases was diagnosed as benign in15cases benign lymph nodes in ultrasound contrast test,1case was diagnosed as lymphoma; two cases of metastatic tumor are right in ultrasound contrast test; There are nine cases was diagnosed as lymphoma in10cases of lymphoma in ultrasound contrast test,1case was diagnosed as benign lymph nodes. CEUS results show that, benign lymph nodes mainly enhance from the central to the whole lymph node (11/15), or from the peripheral to the whole lymph node (1/15), or enhanced disorder (3/15); Metastatic lymph node enhance from the peripheral to the whole lymph node (2/2); Lymphoma mainly enhance from the central to the whole lymph node (8/10), or from the peripheral to the whole lymph node (1/10), or enhanced disorder (1/10)1. The diagnostic accuracy of ultrasound contrast is92%.The comparison of quantitative parameters of the time-intensity curves of three groups of lymph node (Benign, metastatic, and lymphoma lymph node) showed that:there was no significant difference in time to peak (TTP), peak intensity (PI) and speed parameter (β)(p>0.05); The arrive time of Lymphoma is shorter than orther two groups (p<0.05).Conclusion:The CEUS perfusion characteristics of the lymph node diseases provide a valuable basis for the diagnosis of superficial lymph nodes diseases.The observed indexes of the time-intensity curves of all the lymph nodes are of great value in the judgment of benign and malignant lymph nodes. And the arrive time can be used for the judgment of benign and malignant lymph nodes.
