Objective To explore the value of surgical therapy in the treatment of cryptococcal meningitis.Methods The clinical manifestation and therapy procedure of27cases of cryptococcal meningitis patients in our hospital are retrospectively anaylized. All the patents have intracranial hypertension.10patients underwent surgical therapy. We further retrospect the literatures for more in-depth understanding.Results The clinical efficacy rate of the OMMAYA RESERVOIR GROUP and DRUG THERAPY GROUP was83.3%and36.7%respectively. Compared with the DRUG THERAPY GROUP, the OMMAYA RESERVOIR GROUP achieved significant improvement (P<0.05). Although for the patients who have symptom of brain hernia, the external ventricular drainage cannot improve the clinical efficacy rate (0%). it has a trend of prolonging survival time. Conclusion Surgical therapy plays an useful role in the treatment of crvptococcal meningitis, especially for the patients who suffer from intracranial hypertension. OMMAYA reservoir can improve the clinical efficacy rate. External ventricular drainage could save the life of patients who have acute intracranial hypertension, especially when brain hernia forms. Besides, it could be used as transition before OMMAYA reservoir and VP operation. VP operation can prolong survival time of the patients and improve the clinical efficacy rate. All the conclusion need to be supported by a larger number of samples.
