Objective:To analyse the prognosis of the kidney transplant recipients from donors after cardiac death (DCD) meeting international standards and prove the feasibility of the DCD kidney.Methods:Retrospective study the preoperative and postoperative information about diagnosis, treatment and follow-up of24patients accepted DCD kidney transplantation.Results:Mean recipient age was43.6±12.4years. Male/female ratio was16/8. In24cases of kidney transplantation from DCD (including1case for the second kidney transplantation) following up6(2-17) months,16patients’graft function returned to normal (IGF) in postoperative early stage, Scr decreased to130umol/L below in one postoperative week; renal function returned to normal slowly (SGF) in5cases,3cases (12.5%) occurred delayed graft function (DGF), requiring for dialysis treatment during the first week posttransplant, with subsequent recovery of renal function. No primary nonfunction grafts. The second kidney transplant recipient occurred acute rejection (AR) which reversed after methylprednisolone (MP) pulse therapy (500mg×3days) and so far, no chronic rejection (CR) occurred.1case occured hydronephrosis after kidney transplantation and reversed after re-anastomosis to the ureter and bladder. Up to now, human survival and renal survival rates were100%and5patients remain satisfactory graft function more than1year.Conclusions:24cases of kidney transplantation from DCD remain satisfactory graft function in the short term, but the long-term effect needs further study. DCD is a very promising way to solve the kidney shortage of organ transplantation in China.
