Objective:To study the distribution in the normal lumbar disc and the co-expression in herniated lumbar disc about NPY and CGRP, and the expression in herniated lumbar disc about NPY.Methods:To collect a total of50cases of human lumbar intervertebral disc tissue specimens(20of cases normal disc, and30cases of the hemiated disc). To collect randomly complete lumbar disc at10cases cadaver, among L3/4disc3cases, L4/5disc5cases, L5/S1disc2cases. Those will be studied the distribution in the disc about NPY and CGRP. At the same time, the human lumbar nucleus pulposus(NP) tissue(L4/5disc7cases, L5/S1disc3cases) were obtained at another10cases cadaver at random for control group; hemiated NP samples of L4/5or L5/S1were obtained at surgery from30cases patients with lumbar disc herniation for experimental group. Those will be studied the expression of NPY in the disc and the co-expression of NPY and CGRP. All the specimens were fixed and sliced, using immunofluoresence histochemical double-staining technique and Immunohistochemistry and semi-quantitative analysis about NPY and CGRP.Results:①To found a lot of NPY and CGRP Immunofluorescence double staining of nerve fiber to distribute in the annulus fibrosus (AF) outer1/3and few it in AF inner2/3and NP in the normal lumbar intervertebral disc tissue, n=10,x2=116.974, P<0.05;②It is increase significantly in NP of the experimental group than the control group about the NPY and CGRP immunofluorescence double staining of nerve fibers. The experimental group (n=30), X±s=28.8960±5.2352; the control group (n=10), X±S=2.3446±0.2372, t=2.902, P<0.05.③It is increase significantly in NP of the experimental group than the control group about the NPY immunohistochemistry of nerve fibers. The experimental group (n=30), X±S=6.6600±2.31375, the control group (n=10), X±=2.2000±1.25529,t=17.617, P<0.05).Conclusion:①his study demonstrates that NPY and CGRP distribute to the annulus fibrosus (AF) outer1/3part in normal lumbar intervertebral disc, not distribute to AF inner2/3and NP;②A large number of NPY are expression in herniated nucleus pulposus and to found that a large number of NPY and CGRP are co-expression in herniated disc, this may be related to discogenic low back pain and it is clear that CGRP can cause discogenic low back pain;③It is show that the sympathetic nerve fiber distribute the annulus fibrosus (AF) outer1/3part and release the NPY and CGRP in the normal intervertebral disc; the sympathetic nerve has grown into the central nucleus pulposus, and released the NPY and CGRP. The NPY has analgesic role or induced pain role about discogenic low back pain which will late detecte the expression of NPY receptor in disc.
