| [Objective] To evaluate the effects of Hemiarthroplasty and Internal fixation for treatment of displaced femoral neck fracture in elderly persons.[Methods] We searched and internalized for all randomized controlled trials and quasi-randomized controlled trials of Hemiarthroplasty and Internal fixation for curing displaced femoral neck fractures in the elderly by electronically searching MEDLINE(1966to march,2011), EMBASE (1974to march,2011), Cochrane Library(Issue3,2011), CBM, CNKI, WANFANG,VIP (up to march,2011).Manual searching published or unpublished materials and conference papers in Chinese and English and reading references of the correlate articles. The quality of the trials were assessed and extracted by2independent evaluators, and meta-analyses were conducted with the Cochrane Collaboration’s RevMan5.0software.[Result]7published randomized controlled trials involving a total of1537patients were suitable for inclusion in the review. The results of meta-analyses revealed that Hemiarthroplasty curing displaced femoral neck fractures in the elderly can reduce the rate of reoperation after24-36months [RR=0.16,95%CI (0.12～0.22) P<0.00001] and the rate of major complications [RR=0.17,95%CI (0.12～0.23) P<0.00001] compared with Internal fixation of closed reduction.[Conclusion] Compared with Internal fixation of closed reduction, Hemiarthroplasty curing displaced femoral neck fractures in the elderly can reduce the rate of reoperation after24-36months and the rate of major complications.There are no statistical differences between the two ways on the mortality rate of post-operation for12～36months. But,that whether the age, mental conditions of patients are contribution to the relation between choice of the two operations should be also requested multicenter, large scale randomized controlled trials which involved age and mental conditions in the elderly with displaced femoral neck fractures. |
