ObjectiveTo compare the diagnostic value of SPECT-CT and MRI for the senile vertebral fracture, to analyze the correlation between SPECT/CT and "three dialectic phases" that raised in orthopedics of TCM.MethodThis prospective study included the patients over60years who suffer from vertebral compression fracture from October2011to April2012. MRI and SPECT/CT were performed on the patients who agree to take these examinations when contraindication of MRI and SPECT-CT was excluded, then we compare the differences and compliance in diagnosing different types of osteoporotic vertebral compression fractures and evaluate whether both of two methods can locate different type of fractur vertebrae accurately, thus the different sensitivity of discovering simultaneous bone lesion was observed. Patients with osteoporotic vertebral compression fractures who were diagnosed by MRI and SPECT-CT agreed to accept the percutaneous vertebral augmentation, which’s target vertebra was judged by MRI. The three dialectic phases of TCM were classified according to exact duration of disease. All of SPECT/CT images and radiation index were analysed, reference ranges of radiation index of fractures in different dialectic phases of TCM were calculated.ResultThe target vertebras were performed percutaneous vertebral augmentation according to SPECT-CT and MRI, VAS scores of one week and one month after operation were significantly decreased than preoperative score, which showed that the treatment is effective. The value of SPECT-CT is similar to MRI in determining target vertebras for vertebral augmentation procedure. Both of them show favorable correlation in locating the disease vertebras. In primary calculation, the range of radiation index in early stage was (1.5-1.7), intermediate stage was (1.1-1.3) later stage was (0.9-1.1).ConclusionBoth SPECT-CT and MRI show high correlation in diagnosing target vertebras for percutaneous vertebral augmentation. Radiation index counted from fracture vertebras in SPECT/CT might be valuable to make a judgment of different dialectic phases of TCM for senile vertebral fracture.
