522Outpatients With Ra Patients Single-center Clinical Epidemiological Investigation

Purpose:To understand the clinical epidemiological characteristics of outpatient department of our hospital of RA, RA diagnosis and scientific research to provide clinical basis; evaluation of outpatient treatment effect in RA patients; understanding of outpatient mental health.Methods:Using self-made questionnaire, establishing Excel database, using SPSS13.0statistical analysis.Results:1, demographic characteristics:survey of522cases of female342,male180cases, the gender ratio is1:1.9, an average of47.93±13.33years, farmer Occupation:248cases (47.51%),121cases (23.18%) workers, cadre of41cases (7.85%),17cases (3.26%), teacher other and no workers in85cases (16.28%); Nationality:Han in305cases (58.43%),183cases (35.06%), the other minority nationalities in34cases (6.51%); education:high school diploma and453patients (86.78%), college degree and above in69cases (13.22%); regional:195cases (Nanning City37.36%), Chongzuo city in48cases (9.20%),41cases (7.85%) of Guigang city; Income:income less than1000yuan in274patients (52.49%),1000-3000yuan in187patients (35.82%),3000~5000yuan in49patients (9.39%),5000and above in12patients (2.3%);2, clinical features: the onset age of42.13±12.85years, female40.53±11.87years, peaking at35to44years old, men46.15+14.06years, peaking at45to54years, mean8.2 ±6.8years course. The form of onset:mainly for joint pain (97.7%), the first joints commonly followed by proximal interphalangeal joint, wrist, knee joint, metacarpophalangeal joint; the course of joint deformity in255cases (48.9%).For coinfected patients with a total of148cases (28.35%), urinary tract infection in52cases (34.44%), upper respiratory tract infection in32cases (21.19%),24cases of pneumonia (15.89%),21cases of hepatitis B (13.91%), skin infection in14cases (9.27%),5cases of pulmonary tuberculosis (3.31%).The damage of blood system system:216cases (41.38%), urinary system in33cases (6.32%), the digestive system in71cases (13.6%), respiratory system in54cases (10.34%),27cases (5.17%) endocrine system, cardiovascular system in44cases (8.43%), reproductive system in7cases (1.34%),6patients with cutaneous vasculitis (1.15%), popliteal cyst in6cases (1.15%), rheumatoid nodules in23cases (44.06%), associated with Sjogren syndrome in35cases (6.7%).The immunological examination:ANA was positive in152patients (29.12%), wherein the homogeneous type in85cases (55.91%), particle type in39cases (25.66%), cytoplasmic type in17cases (11.18%), kinetochore type in6cases (3.95%), nucleolar type in5cases (3.29%); the patients with positive AKA in233cases (44.6%); anti CCP antibody positive in439cases (84.09%), as a drug:MP (95.79%),21cases (3.63%) with prednisone, methotrexate (97.89%),511cases of leflunomide in409cases (78.35%), hydroxychloroquine in157cases (30.08%), Lin in27cases (5.17%),51cases (Tripterygium wilfordii9.77%), Zhengqing Fengtongning in198cases (37.93%), biological agents in15cases (2.86%). He target therapy:418cases of patients with newly diagnosed mild activity level,7people (1.68%), severe activity70people (98.32) reported remission;134(32.06%), mild activity82(19.62%), severe activity123people (48.32%).More than half (51.62%) to achieve treatment goals, joint pain:the patient with newly diagnosed VAS score and score were8.32±3.16,4.33±2.08(P&It;0.01), physician ratings were6.88±2.17,4.12±1.84(P&It;0.01).In3, the cost of treatment:first diagnosis:an examination of Fei Pingjun1166.24±276.012yuan, an average of402.82±70.295yuan fees, travel of202.43±57.61yuan, referral (monthly):examination fee of87.44±60.124,385.85±92.102Yuan Yuan fees, travel of83.5±64.51yuan.4, anxiety, depression:investigations on260patients were investigated, depressive patients in67cases (25.77%):female (33.94%56/165), college degree or above in15cases (46.88%15/32), a joint deformities (35.71%35/98) are more vulnerable to depression; anxiety patients70cases (26.92%) female:(33.33%55/165),37cases of patients with no health insurance (34.91%37/106), college degree or above (46.88%15/32), joint deformity in patients (34.69%34/98) prone to anxiety.SDS, SAS standard were46.53+13.24,41.31+11.99, divided, and domestic often die than the differences were statistically significant; depression and anxiety symptoms and joint function, disease activity correlated positively, negatively correlated with age.Conclusion:1, my courtyard outpatient service in RA patients demographic characteristics:farmers, low education, low income patients, male proportion increases.2clinical features:age of onset of female40.53±11.87males aged46.15±14.06years, long course of disease, high incidence of infections, a common system damage.In3, RA in patients with anxiety and depression incidence is high, should pay attention to the psychological condition of patients, patients with maintenance of mental health.
