Objective:Chronic obstructive pulmonary disease (COPD) is a kind of disease with airflow limitation characteristic, airflow limitation is not fully reversible, and also shows a progressive development COPD closely relates with an abnormal inflammatory response of the lungs to noxious particles. In traditional Chinese medicine (TCM), COPD is considered as the scope of "Asthma","lung inflation". The current study is to investigate the correlation between lung surfactant protein D (SP-D) and TCM diagnoses of COPD including phlegm heat Yun-lung, lung and kidney qi deficiencies, explore and discover the material basis of TCM diagnosis of COPD, to provide some complementarity for TCM diagnoses, raise the level of TCM diagnosis. Methods:According to national standards "Chinese clinical diagnosis and treatment terminology GB/T16751.2-1997",49patient including16cases with the acute exacerbation of COPD of phlegm-heat retention,17cases with stable COPD of qi deficiency syndrome, kidney16cases with deficiency syndrome of stable COPD, and14cases of healthy volunteers were involved in current study. Blood was collected from each subject and serum was stored at-80℃until the samples were detected. Serum SP-D levels were analyzed with the enzyme-linked immunosorbent assay (ELISA) method; lung function data of stable COPD qi and kidney deficiencies groups were also acquired.Results:1. Compared that in healthy volunteer group (18.18±5.87), the serum SP-D levels in COPD with phlegm-heat retention (25.22±8.84, P<0.05) and kidney deficiency group (24.84±13.39, P<0.05) were significantly higher;2. No statistical difference was observed on serum SP-D levels between phlegm-heat retention group and kidney deficiency group (P>0.05);3. The serum SP-D levels in phlegm-heat retention and kidney deficiency groups were higher than that in the lung qi deficiency group (17.21±6.09,<0.05);4. The difference of serum SP-D levels between lung qi deficiency and the healthy group showed no statistical significance (P>0.05);5. Lung functions FEV1between kidney deficiency and lung qi deficiency groups were significantly different (P<0.01).Conclusion:Some correlation was showed in the serum SP-D levels between different COPD TCM types; detection of serum SP-D level together with lung function level measurement, could be used as a reference for the diagnosis of clinical syndrome of COPD in TCM.
