To Analysis the clinical significance of troponin autoantibodies in patients withacute myocardial infarction.Methods:Between April2011and March2012,68patients confirmed AMI in ourdepartment were taken venous blood for the test of troponinI and troponin autoantibodies titer whilediagnosis was confirmed and within24h after diagnosed according to the test results, all cases aredivided into troponin autoantibody-positive group (13positive group) and troponinautoantibody-negative group (55negative group). Comparison between the two group is made on thetroponin levels while diagnosis was confirmed,the troponin levels within24h after diagnosed,and theimmediate angiography results. To analyze the influence of positive troponin autoantibodies to thelevel of troponin and the relationship between positive troponin antibodies and the severity of coronaryartery lesion; To compare the left ventricular ejection fraction (LVEF),left ventricular end-diastolicdiameter (LVEDD),left ventricular end systolic diameter (LVESD) at7days and180days afterdiagnosed; and to evaluate the relationship between troponin autoantibody and the changes ofcardiac function of patients with AMI in short-term period and long-term period.Results:1)The levels of Troponin autoantibody positive group while diagnosed are lower than the negativegroup,However,there was no statistically significant difference,and24h after diagnosed the troponinlevels are significantly higher than that of negative group (P <0.01).2) In the positive group, the probability of multivessel disease is much larger than the negativegroup. The Gesini integral and SYNTAX points of the positive group are higher than that of thenegative group, difference were statistically significant (P <0.01).3) At7days after diagnosed, the BNP level of Troponin autoantibody-positive group issignificantly higher than that of negative group at7days after diagnosed (P <0.01). the left ventricularremodeling index LVEDD and LVESD of positive group are higher than negative group (P <0.01),and EF is lower (P <0.05).4)180days after diagnosed, the BNP levels of positive group is higher than that of negative group. left ventricular ejection fraction (EF),left ventricular end-diastolic diameter (LVEDD),left ventricularend systolic diameter (LVESD) at180days after troponin autoantibody-positive group (Igroup) inpatients with heart function is still worse than that of troponin autoantibody-negative group.Conclutions:The Troponin autoantibody may influence the concentration of troponini atearly stage of AMI, which can interfere with the early diagnosis of acute myocardial infarction andevaluation of cardiac function. Preliminary observations from the long-term followup visit showthat,the presence of troponin autoantibodies may affect the postoperative recovery of cardiacfunction in myocardial infarction.
