Objective:①To explore the clinical significance of the variation of CK,CK-MB,the ratio of CK-MB/CK,cTnI in the patient suffered from ACVD and make a comparative analysis between ACVD and AMI.②To study the relation of the variation of CK,CK-MB,the ratio of CK-MB/CK,cTnI and disease type, the severity,the level of conscious disturbance and cardiac complication in ACVD.Methods:To choose 62 patients with ACVD as the study group,30 patients with AMI as the control group.All the patients should be taken venous blood in 24h,3d,7d to determine CK activity,CK-MB activity.The level of cTnI,to calculate the ratio of CK-MB/CK,simultaneous recording ECG,to observe the signs and symptoms of the patients(consciousness,blood pressure,heart rate,chest pain/radiating pain,cardiac status).If it is possible the patients maymake the heart ultrasonic inspection,observe the value of EF and ventricular wall motion.Results:In the study group,2 cases were eliminated for AMI.The abnormalityrates of CK,CK-MB,cTnI were 45%,18.33%,56.67%in 24h in 60 patients.There were 32 patients with abnormal ECG.The abnormality rateis 53.33%, 2 patients with AMI.The abnormality rates of CK,CK-MB,cTnI and ECG were all 100%.1,The levels of CK,CK-MB,cTnI were significantly lower in study group in 3 time points compared with the control.The ratio of CK-MB/CKwas lower than the control(P<0.05).2,The levels of CK,CK-MB,cTnI between ACI and ACH weren't significantly different in study group in 3 time points(P>0.05).The ratio of CK-MB/CK was lower in ACI than in ACH patients(P<0.05).3,The levels of CK,CK-MB and the ratio of CK-MB/CK were significantly higher in ACVD patients with myocardial injury than in ACVD patients without myocardial injury(P<0.05).4,The levels of CK,CK-MB and the ratio of CK-MB/CK were significantly higher in ACVD patients with abnormal ECG than in ACVD patients withnormal ECG(P <0.05).The comparison of the level ofcTnI in 3 time pointswasn't statistically significant(P>0.05).5,The levels of CK,CK-MB in ACVD patients with consciousness disturbance were significantly increased compared with those without consciousnessdisturbance(P<0.05).The comparison of the level of cTnI in 3 time pointswasn't statistically significant(P>0.05).Conclusion:1,Serum myocardial zymogram in acute stage in some ACVD patients were higher than normal level,but were low compared with AMI patients.2,The variation of CK,CK-MB,cTnI were independent on the type of disease in ACVD,and correlate with the degree of consciousness disturbance.3,ACVD patients with myocardial injury or AMI usually show abnormality with ECG and Serum myocardial zymogram in clinical diagnose,cTnI was the best determine biochemical marker in ACVD patients with myocardial injury.The abnormality of CK,CK-MB help to diagnosis,especially the ratio of CK-MB/CK>5%.
