ObjectiveTo observe the therapeutic effect of thermal moxibustion therapies ofGuanyuan point on menstrual symptoms after confirmation its effectivenesson primary dysmenorrheal. Using BOLD-fMRI technology observes thebrain regional homogeneity changed characteristic before and aftermoxibustion Guanyuan and detects the brain effects and central nervoussystem mechanism of patients with primary dysmenorrheal from the view ofImageology.Method1.104patients with primary dysmenorrheal were randomly dividedinto the moxibustion Guanyuan group(Guanyuan group) and moxibustionShenque group(control group).Each group include52patients and theywere divided according to their menstrual symptoms:abdominalbulge,backache,breast tenderness,diarrhea.2.Moxibustion treatment were performed4days before menstruatingfor4days,once a day.Each moxibustion treatment lasted20minutes. Atotal of3Menstrual cycles were included. 3. Refer to the Diagnostic efficacy of standard TCM Syndrome and theCox Menstrual Symptom Scale。To evaluate the change condition ofpatients before and after treatment. To observe the clinic effects ofmoxibustion-Guanyuan in the patients with primary dysmenorrhea and theimprovement of common simultaneous phenomenon in menstruation4. There were20primary dysmenorrhea-eligible patients undergoingresting-state BOLD-fMRI scanning before treatment (moxibustion thepoint of Guanyuan) and after5minutes when treatment was completed.ReHo and t-test were used to process the20subjects dataset.Results1. Therapeutic effect of Clinical Observation1.1The effective rate of Guanyuan group was90.39%and that ofcontrol group was71.15%.There was significant difference betweenGuanyuan group and control group(P<0.05).1.2Dysmenorrhea scores after treatment was significant differentfrom that before treatment in both group (P<0.01)and the improvementsituation of Guanyuan group was better than that of control group(P<0.01).1.3The effective rates of abdominal bulge,backache,breast tendernessand diarrhea of Guanyuan group were respectively91.30%,94.12%,16.67%,86.96%,and those of control group were58.33%,65.52%,11.76%,50.00%.Compared with control group,thetherapeutic effect of abdominal bulge, backache, diarrhea were better(P<0.01)and that of breast tenderness was not different(P>0.05).2Changes of brain areas before and after treatmentReHo values increase significantly in the right superior temporal gyrus,the left middle temporal gyrus, the right anterior cingutate, the leftcingulate gyrus,the right middle frontal gyrus, the left medial frontal gyrus,the right postcentral gyrus and the left precentral gyrus after treament.(P<0.05, after correction).However, ReHo values decrease significantlyin the right cuneus, the left midbrain, thalamus and the left occipital lobe.(P<0.05, after correction)Conclusion1.Moxibustion Guanyuan point can effectively treat primarydysmenorrheal.2.Moxibustion Guanyuan can effectively improve menstrualabdominal bulge, backache, diarrhea symptoms of patients with primarydysmenorrhea, but has no significant treatment effect on breast tenderness.3. Moxibustion the point of Guanyuan plays an important role in theincreasing or decreasing of ReHo value in a series of pain-related encephalicregions.These encephalic region may be the target of analgesic effect ofmoxibustion.That may be the conceivable central analgesic mechanism ofmoxibustion the point of Guanyuan for patients with primary dysmenorrhea.
