Obejective:TO study the serum concentration‘s dynamic changes of heart-type fatty acid-bindingprotein(H-FABP) in the early stage of ACS, and compare the serum concentrations of H-FABPwith high-sensitivity C-reactive protein (hs-CRP) and cardiac troponin I (cTnI); in order torealize the concentration of H-FABP with different types in ACS and different time periods inAMI, to investigate their clinical significance.Methods:A total of83acute coronary syndrome patients who were in hospital within12hours fromonset. Each patient made a definite diagnosis by clinical symptoms and coronary angiography(CAG) and so on. In addition, we set up stable angina pectoris (SAP) group of30cases andnormal control group of30cases. Each patient made a definite diagnosis by CAG and takenmorning fasting blood samples. The patients with acute coronary syndrome divided intounstable angina pectoris(UAP) group (n=48) and acute myocardial infarction(AMI) group(n=35), whose taken hospital admission instant blood. AMI group divided into <3h group,3-6h group and≥6h group. We tested the serum concentration of H-FABP, hs-CRP and cTnIwith quantitative detection method, In all groups, compared each test index the difference andthe change we also observed in the occurrence of major adverse cardiac events(MACE) in allgroups within3months’ clinical follow-up.Results:1. The serum concentrations of H-FABP, hs-CRP and cTnI were much higher in theAMI group than UAP group, SAP group and control group (p<0.01); the serumconcentrations of H-FABP, hs-CRP were much higher in the UAP group than SAP group andcontrol group(p<0.01), but there were no difference in cTnI;.the serum concentrations ofH-FABP was much higher in the SAP group than control group(p<0.01), but there were nodifference in hs-CRP and cTnI.2. The serum concentrations of H-FABP, hs-CRP were much higher in the AMI <3hgroup than UAP group, SAP group and control group (p<0.01); but there were no difference in cTnI.The serum concentrations of H-FABP, hs-CRP and cTnI were much higher in theAMI3-6h group and AMI≥6h group than UAP group, SAP group and control group(p<0.01).3. The serum concentrations of hs-CRP, cTnI were higher in the AMI≥6h group thanAMI3-6h group and AMI <3h group (p<0.01), but there were no difference in H-FABP;Theserum concentrations of cTnI were higher in the AMI3-6h group than AMI <3h group(p<0.01), but there were no difference in H-FABP and hs-CRP.4. The serum concentrations of H-FABP was positively correlated with the concentrationsof hs-CRP and cTnI (p<0.001).5. Following-up results: During the follow up period,there were ten patients occurredMACE, included myocardial infarction two, recurrence of angina pectoris seven and deathone.In ten cases of MACE,nine cases in ACS group, one case in SAP group. MACE occurredin those case whose serum concentrations of H-FABP, hs-CRP were markedly elevated.Conclusion:1. H-FABP may be an valuable predictor of ACS,can be used in the early diagnosis ofACS patients.2. H-FABP may be risk factors for the patients in ACS.
