Abstract/Summary:
Objective1.To build3-D finite element model of Crowe â…¢ adult developmental dysplasiaof the hip. And then the acetabular prosthesis was fixed at the true and fake acetabularposition simulated under tolal hip replacement. Using finite element method, comparethe mechanics difference in different position. Provide a theoretical basis for Croweâ…¢adult developmental dysplasia of the hip in the surgical options.2. To follow-up of the Crowe â…¢ adult developmental dysplasia of the hip treatedby tolal hip replacement and to evaluate the clinical outcome.Methods1. One patient of Crowe â…¢ adult developmental dysplasia of the hip choosedwith no other disease to make64-row spiral CT scan on the pelvis and proximalfemur and achieve data for building3-D finite element models by using Minics10.01and Abaqus6.9. fixing the3-D finite element model of acetabular prosthesis byimporting geometric data of the artificial prosthesis at the true and fake acetabularposition simulated under total hip replacement. After that, get the stress distribution,stress and displacement values by setting boundary and loading conditions, then dosome biomechanical analysis.2. To reviewed32cases of Crowe â…¢ adult developmental dysplasia of the hiptreated by total hip replacement between January2006and february2011in ourhospital.24cases(24hips) were followed up with an average of47.5months(range,15-75months) including10male and14female with an average age of53.5yearsold (range,36-75years old). The clinical outcome was scored with Harris methods,the prosthesis whether loosening or fracture showed by hip X-ray films. Results1. By the finite element evaluation, the stress and displacement values of thetrue acetabular position fixed by acetabular prosthesis in3-D finite element model ofCrowe â…¢ adult developmental dysplasia of the hip are smaller than the fakeacetabular position and the stress distribution are more close to the normal acetabular.2. Through the analysis of24cases of Crowe â…¢ adult developmental dysplasiaof the hip, clinical outcome is satisfactory, the average Harris scores increase frompreoperative26.65(range,10-55) to87.5(range,76-97) at last follow-up, no prosthesisloosening were observed.Conclusions1. For the Crowe â…¢ adult developmental dysplasia of the hip, acetabularprosthesis fixed on the true acetabular position is better than the fake acetabularposition treated by tolal hip replacement.2. In treatment of Crowe â…¢ adult developmental dysplasia of the hip, THRwith acetabular prosthesis fixed on the true acetabular position provide satisfactoryshort medium-term effectiveness.
