Background WHO proposed the concept of essential drugs system in the1970s, which aimed at improving the essential drugs’availability for the poor to promote the rational use of drugs.The concept has been gradually recognized by most countries of the world,and it has been proved that less medical and health expenses can exchange for a greater population health gains.China has also introduced the concept of essential drugs earlier, and published the first edition of essential drugs dictionary in1982.However, not developed to improve the comprehensive policy, the implementation of the system did not achieve the desired objectives because the relevant policies were not formulated and improved.As an important measure to solute the "expensive medical treatment" problem,China clearly proposed to improve the state’s essential drugs system as one of the recently five key reforms in the deepen meidical and health care system reform,and made greater efforts. The essential drugs system first started in the government primary health care institutions (township health centers and urban community health service organizations) in early2010, and has got the full coverage of primary health care institutions by the end of2011.The following efforts have been taken since the startup of the system. First, re-selection to determine the national essential drugs dictionary to reducie the variety of essential drugs. Second,improve the financial compensation of primary health care institutions and medicare reimbursement for essential drugs to provide the essential drugs funding for the masses.Third,the government reduce the price of essential drugs by re-pricing,bidding and purchasing,no-profit saling of essential drugs and other measures.Four,primary health care institutions are required to equip with and use only essential drugs to re-build the new service model.Five,increase advocacy and training efforts to improve the level of rational use of drugs.Through two years of practice, essential drugs system has played a certain degree of success in improving the functional position of the primary health care institutions and solving the grassroots masses’expensive medical treatment and so on.With the extention of the system coverage for village and city health and medical institutions,the beneficiary population will alsofurther expanded.However,the essential drugs system, after all, is a new thing,and there are still some problems and shortcomings in the advancing process. This paper intends to combine the actual situation in Shandong Province to discuss the building of essential drugs system and supporting system.Objectives To describe the government strategy on implementation the essential drugs system by summarizing the policies of essential drugs system and related system introduced by the Central and Shandong Provincial Goverment system.Meanwhile, according to the conventional data monitoring of the Division of Drug Policy and Essential Drugs System, combined with the actual operation of primary health care institutions, to give qualitative and quantitative evaluation, find the problems during the operation of essential drugs system in Shandong Province, and find the targeted recommendations.Research contents1. It uses the macroscopical policy research method to make systematically classification and description about the essential drugs directory settings, the performance salary reform, financial compensation and so on.2. The achievement in the institution incomes, performance salary reform and on-line procurement after the implement of the essential drugs reform.3. Problems relating to the primary health care institutions after the implement of the essential drugs policy and policy suggestion according to the actual process. Conclusion and suggestion1.There are great changes in the drug benefits, disease burden and medical insurance for essential drugs referring to the essential drug policy.2. There are still obvious insufficient in the supporting reform mainly including the directory setting, local finance input and salary reform etc.3. The key areas of future work should focus on the pharmaceutical reform in the public hospitals and village clinics, procurement and distribution of essential drugs, medical service price reform and other aspects.
