ABSTRACTBackground and Objective:Breast cancer is of the highest incidence among all the female malignant tumors at present, which poses a serious threat to the wemen’s health and life. Compared with the European and American countries, China is a country with lower incidence of breast cancer. However, in the recent20years, the annual growth rate of the morbidity of breast cancer has been1~2%higher than developed countries, and it continues increasing by3~4%annually. The morbidity has been showing a significant rising trend. There are many internal and external factors leading to breast cancer. The significance of the relationship between obesity and female breast cancer has gradually been realized with further epidemiological researth which concerns the risk factors of breast cancer. In order to improve the effect of treatment and the prognosis of the breast cancer patients, early prevention is also crucial we develop the medical technology at the same time. In Western Europe, North America and such countries with high incidence of breast cancer, the researches on the relationship between breast cancer and body shape indices have been numerous and relatively thorough. However, currently, the domestic research of this aspect still lacks and the existing results are of many discrepancies. Clinical study is adopted here, to discuss if there are some differences between patients with benign and malignant breast diseases in body shape indices such as BMI(body mass index), WHR(waist hip ratio), HHR(hip height ratio), circumference of chest, waist and hips, expecting to have a directive function in the prevention of breast cancer.Methods:A clinical study was adopted. There were183female patients who were diagnosed with breast cancer by pathology at Department of breast surgery of Qilu Hospital of Shandong University from March,2010to May,2011, in breast cancer group, and182female patients with benign breast diseases in benign group. The clinical and pathological data and body shape indices of the both groups were analyzed.Results:(1)The average age of the benign breast diseases patients is (39.71±10.55) years old, while the average age of the breast cancer patients is (50.32±11.04) years old(P=0.000<0.05). In general terms, the patients with breast cancer are elder.(2)The youngest age of the patients with breast cancer is30years old. And the incidence increases with age, which reaches its peak level at40to49years old (about39.4%), and falls with age after menopause.(3)The average menopause age of the benign breast diseases patients is (50.17±3.92) years old, while the average menopause age of the breast cancer patients is (46.37±7.44) years old(P=0.043<0.05). The patients with breast cancer went into menopause in an elder age. And the premenopausal breast cancer accounted for57.38%of breast cancer.(4)Most breast cancer is hormone-dependent.(5) In the study, the average values of circumference of chest, waist and hips, weight, BMI, WHR, HHR of breast cancer patients are larger than those of benign breast diseases patients(P circumference of chest、Pcircumference of waist、Pweight、PBMI、PWHR、PHHR=0.000<0.05, P circumference of hips=0.001<0.05)), while the average value of height of breast cancer patients is lower (Pheight=0.006<0.05).(6)There is no obvious correlation between breast cancer and body shape indices such as circumference of chest, waist and hips, height, weight, BMI, WHR, HHR etc, in all kinds of ages or menstrual state.(6)Comparing with each index of two groups with different expression of ER and PR, WHR of the premenopausal breast cancer patients with ER-PR-is larger than that of benign breast diseases patients(PWHR=0.045<0.05). BIM, HHR and the circumference of hips of the postmenopausal breast cancer patients with ER+PR+are significant larger than those of benign breast diseases patients(P circumference of hiP=0.029<0.05 PBMI=0.015<0.05, PHHR=0.007<0.05), while height is relatively lower than those of benign breast diseases patients(Pheight=0.046<0.05). It does not suggest that there is no correlation between female breast cancer and body shape indices. It is just that the correlation is limited to breast cancer with a certain expression of ER and PR. So, in order to prevent breast cancer, it is important to eat a healthy balanced diet after menopausal, practice good health habits and lead a healthy lifestyle to control weight and improve the body fat distribution.Conclusions:(1)In the365hospitalized patients, the average values of circumference of chest, waist and hips, weight, BMI, WHR, HHR of breast cancer patients are larger than those of benign breast diseases patients, while the average value of height of breast cancer patients is lower. But the difference is not significant considering the factor of age.(2) Most hormone-independent premenopausal breast cancer (ER-PR-) patients are those of central obesity (larger waist-hip ratio).(3) Most hormone-dependent postmenopausal breast cancer (ER+PR+) patients are those of pycnic type(larger circumference of hips and BMI, but lower height).(4) Hip height ratio is more sensitive than a single index such as height or circumference of hips. It is expected to become a new index to evaluate the body shape of breast cancer patients in clinical work.
