Objective:To explore the indications and superiority of improved Wiltse treating with single lumbar segmental degenerative disease using unilateral pedicle crew fixtion and interbody fusion technology through clinical application case analysis, and provide theoretical basis for clinical works.Method:31patients who has been treated with single lumbar segmental interbody fusion technology were divided into two groups randomly, all the patients were hospitalized during2010to2012.14patients were treated by minimally unilateral fixation fusion, and the mean follow-up time is15.2months. The other17patients were treated by bilateral fixation fusion, and the mean follow-up time is16.1months. Compare the pre-operation JOA points, the operating time, the amount of bleeding, the time stayed in bed, the days in hospital, the cost in hospital, the JOA points at the last follow-up time, the rate of interbody fusion and the complication situation between the two groups.Results:There is no Statistical differences in the pre-operation JOA points, the OA points at the last follow-up time, the rate of interbody fusion and the complications between the two groups (P>0.05). All the patients got better function after operation. But the operating time, the amount of bleeding, the time stayed in bed, the days in hospital and the cost in hospital had been decreased by using improved Wiltse method.Conclusion:For the patients who have single lumbar segmental degenerate and lumbar instability and exist with surgical indications, using improved Wiltse method can get the same effect which treating with bilateral fixation fusion. And the method can decrease the operating time, the amount of bleeding, the time stayed in bed, the days in hospital and the cost in hospital. This kind of method is worthed in clinical promotion.
