objective:To evaluate the therapeutic effect and possible mechanism of propranolol on the proliferating hemangioma. Methods: To use the freshly resected tissue of infant proliferating hemangioma transplant to nude mouse to set up angiomatous animal model. On the45day of transplant,dividing randomly the live transplant nude mouse into two groups:Group1,which is the therapy group of using propranolol group2, the control group using physiological saline.Every group has11mouse and perfusing separately propranolol and physiological saline to therapy and then to observe the growing status respectively. On the15after the medical interference, killing the experimental nude mouse in the way of dislocating tocervical vertebrae to resect the transplant tumor tissue. To detect Vascular endothelial cells MVD of transplant tumor tissue, the expression of CD34、Ki-67、Bcl-2with the use of immunohistochemistry. To detect Apoptosis in transplanted tumor tissue in the TUNEL method and the Changes in the concentration of cAMP and PKA activity with the use of enzyme-linked immunosorbent assay. Results:1. Transplanted tumors observed in general and light microscopic pathology:after1week, Significant difference of transplanted tumor volume appears between the propranolol group and the saline group(P<0.05); In the drug intervention after2weeks, the propranolol group transplantation tumor volume was significantly smaller than the saline group, the two groups had significant difference (P<0.01). Transplant tumor in propranolol group is under the pale color, in poor blood supply, obviously smaller and the texture hardens. In the light microscope, in the propranolol group the tumor tissue is mainly made of fat and fibrous tissues. The Vascular endothelial cells is in sparse dispersed, the vessel wall deformates and collapses, the structure is destructed. And in the physiological saline group,hyperplastic capillary vessel richly and the vessel wall have not been destroyed.2. Immunohistochemistry:The expression of MVD value, CD34、Ki-67、 Bcl-2、VEGF of the propranolol group are obviously lower than the physiological saline, the two groups have clearly difference(P<0.05). Apoptosis index between the propranolol group and saline group are significantly different with the use of TUNEL method(P<0.01).3. In Enzyme-linked immunosorbent assay the concentration of cAMP and PKA activity in the propranolol group was significantly lower than the saline group(P<0.05), and the concentration of cAMP and PKA activity in the propranolol group was significantly positively correlated(y=0.607, P<0.01). Conclusions:1. Propranolol has significantly therapeutic effect on proliferating hemangioma.2. Propranolol can induce the apoptosis of vascular endothelial cells and inhibit their proliferation and promote tumor’s subsidise.3. Propranolol may control the activation of the ERK/ MAPK signaling pathway through inhibition of the cAMP/PKA signaling pathway, thus inhibiting the secretion of VEGF of proliferating blood vessel.
