As a group of cutaneous symptoms caused by excessive androgen in women, the SAHA syndrome is clinically characterized mainly by seborrhea,acne,hirsutism and androgenetic alopecia.The acne can occur on many areas of the body in different degrees of severity with a variety of forms. Hirsutism often occur later, it refers to the excessive growth of terminal hair in women,with distribution and growth characteristics like men. Androgenetic alopecia usually begins in adolescence.It’s a type of telogen effluvium,usually in Ludwig mode.The hair will gradually become diffuse and tiny,with forehead hairline unchanged.The most obvious area is the top of the head, followed by coronary area.Each of the main symptom affects the appearance,put great mental pressure and psychological burden on women. Severe cases can be associated with multiple endocrine diseases, and even malignant tumor, pose threatens to patient’s health and lives.At present,little attention has been paid to the syndrome in China.Initial or main symptoms are often too focused in clinical work.Failing to conduct a comprehensive assessment and diagnosis will result in missed diagnosis. Knowing the incidence, characteristics and laws of the sybdrome is necessay for the correct diagnosis and effective treatment.Research ObjectiveKnowing about the incidence of SAHA syndrome, having a more comprehensive understanding on clinical features, laboratory features,and influential factors of the syndrome in Chinese Han population, and factors have a, exploring the TCM Typing characteristics, Provide a basis for its diagnosis and TCM treatment.MethodBy surveying patients at Nanjing Medical University affiliated Friendship Plastic Surery Hospital dermatology outpatient department with the designed survey, medical records were collected using the study selection criteria included and excluded,finaly retained350records.The severity of acne, the presence of alopecia or hirsutism, and the TCM type were estimated by physicians.According to the sex of the patient and SAHA syndrome diagnostic criteria, the patients were divided into three groups:the SAHA syndrome group, the usual female acne group,the male acne group. All the data is inputted into the computer and analysis by SPSS17.0statistics software.ResultIn350patients with acne.women accounted for78.86%.45.49%of the female patients are diagnosed with SAHA syndrome, only3.6%of them are typical. IN the41patients with syndrome SAHA accepted B ultrasound examination,we found7with ovarian SAHA,1with high PRL SAHA.96.38%of female patients complained acne chiefly in their first visits.others with hirsutism. Hirsute patients scores5-9accout for the most.The most common hairy areas is upper lip, followed by abdominal, lower abdomen, upper back, chin and cheek side, rarely seen in the chest, lower back, upper arms and thighs, often accompanied by hirsutism in androgen sensitive areas. Acne can be widely distributed in the sebaceous glands intensive site, mainly to moderate severity.The ratio of acne patints whose onset age are under20years old is higher in SAHA group than usual female froup.The secerity of acne is lower in SAHA patients than in male patients.The SAHA group is more affectes than the male group.The two female group have similar regularity of menstrual cycle. The test results of sex hormones are similar in SAHA and uaual female group, the SHBG levels are a little lower,as the DHEAs levels are a little higher in the normal range.The male group is mainly constituted by the plegm and blood stasis syndrome,as SAHA group is mainly disharmony of Chong and Ren syndrome.SAHA syndrome group of age of onset for patients under20years higher than the proportion of female acne vulgaris group, acne severity was less than that of the male, affected by genetic effect is larger than the male and female patients with acne vulgaris, the menstrual cycle regularity difference. SAHA syndrome group and normal female acne patients of T, E2, SHBG, DHEAs in serum examination results for the overall distribution differences, SHBG are within the normal range on the low side, DHEAs were within normal limits for high. Acne in men with phlegm and blood stasis syndrome group condensation, and SAHA syndrome group to Chong and Ren Disharmony Syndrome mainly.In SAHA syndrome patients,31.1%of them have less sleep time or poor sleep quality,66.4%of them have more severe acnes when eat spicy,sweet,or fried food,75.2%of them may occur more acnes under pressure,72.8%of them use computer more than4hours each day,66.4%of them have aggravation of acnes.inthe menstrual period or before,after the period.Mild acne patients has higher quality of life than moderate and severe acne patients.There is no difference in life quality between moderate and severe acne patients.ConclusionIn this research.patients with SAHA syndrome accounted for45.29%in the female patients,3.6%of them were tipical.The results implied that SAHA syndrome is common among acne patients in dermatology outpatient department. Correct diagnosis should be made through serious diatingushing according to its clinical and laboratory features. Specific analysis of TCM type and influential factors should be take between treatment.Targeted instructions for acquiring good lifestyle,balanced diet.and positive attitudes,should be given to patients.
