The Effect For The Degeneration Of Patellar Cartilage On Patella Reserving TKA

Background:Osteoarthritis (OA) is a kind of chronic arthritis disease, which is characterized by the degeneration and loss of articular cartilage, bone regeneration on the edge of joint and subchondral bone. The original site is cartilage. It’s the most commom etiology in the inflammatory of knee.There are50%of senior citizens with performance of osteoarthritis in the X-ray films. People’s daily life is affected seriously by osteoarthritis of knee joint. The appearance of total knee arthroplasty(TKA) becomes a suitable therapy of osteoarthritis of knee joint.However, the curative effect of TKA is affected by anterior knee pain.Patellar resurfacing is choosed to solve this problem.But there is still debate about whether patellar resurfacing is necessary. Patellar resurfacing can not be considered during initial TKA in a lot of studys,but several scholars says that patellar resurfacing is necessary when the degeneration of patellar cartilage is bad.Purpose:Through the study of the effect for the degeneration of patellar cartilage on patella reserving TKA,this article provides a basis of whether doing patellar resurfacing.Research methods:collect45knees who has the clear diagnosis of osteoarthritis of knee joint during2011.4～2.12.2in Changzhou Hospital of Traditional Chinese Medicine,make the preoperative VAS score and KSS score.People with patellar cartilage of Outerbridge grading I～III are formed Group Low-damage,named Group A.People with patellar cartilage of Outerbridge granding IV are formed Group High-damage,named Group B.Make VAS score and KSS score after one month,three months and six months of TKA.All the scores carries on statistics processing.Results:There are significant differences in VAS and KSS before and after TKA in group A and B(P<0.05); there is no statistical significance in VAS and KSS between group A and B(P﹥0.05).Conclunsion:1. The degeneration of patellar cartilage has little significant effect for the curative effect of patella reserving TKA in short-term,the long-term effects still need further observation.2. Patellar resurfation can not be considered during TKA,we will obtain the ideal functional recovery of knee joint with patella plasty,denervating patellar peripheral,regulating the lateral tension of patellar,and injecting soft tissue aroune knee joint with analgesic drugs.3. TKA can relive the pain of knee joint OA,and improve joint function effectively.
