Objective To investigate the correlation between the degree of patellar cartilage degeneration and anterior knee pain in patients with knee osteoarthritis after TKA,and to analyze the causes of anterior knee pain after TKA.Methods The clinical data of 83 patients(n = 102)with knee osteoarthritis who underwent total knee arthroplasty from December 2015 to February 2017 in our hospital were collected.Among these,12 were male(n = 14)and 71 were female(n = 88),with a median age of(65.8±7.6)years.The position of patellar cartilage degeneration was observed during operation,and the grade of patella cartilage degeneration was evaluated by the Outerbridge classification.The patients with grade?were Group A1,and the patients with grade?~? were Group B1;And then the patients whose lateral patellar degeneration reached grade IV were Group A2,the patients with grade?~? were Group B2.Preoperative and postoperative 1st,3rd,6th,and 12 th months,VisualAnalog Scores and pain points were recorded;At the preoperative and final follow-up,the Knee Society Scale and Feller Patellofemoral Score were performed.Results All patients were followed up for 2~17 months,with an average follow-up of 8.5 months.The patellar chondromalacia: grade? in 12 cases,grade ? in 11 cases,grade ? in 18 cases,and grade ? in61 cases.Patellar lateral cartilage degeneration: grade ? has 36 knees,grade ?~?has 66 knees.At 6 and 12 months after surgery,the incidence of anterior knee pain showed no significant difference(P>0.05);and there was no significant difference in Knee Score,Function Score,Feller Score and Visual Analog Scores before and after operation between Group A1 and Group B1(P>0.05).At 12 months after surgery,the incidence of anterior knee pain in Group A2 was higher than that of Group B2,and the difference was statistically significant(P<0.05).At 6 months after surgery,the incidence of anterior knee pain showed no significant difference(P>0.05);And there was no significant difference in Knee Score,Function Score,Feller Score and Visual Analog Scores before and after operation between Group A2 and Group B2(P>0.05).Postoperative Visual Analog Scores,Knee Score,Function Score,Feller Score were significantly improved compared with the preoperative,and the difference was statistically significant(P<0.01).Conclusion(1)Whether the patellar cartilage degeneration reach grade IV or not,there was no significant difference in the incidence of anterior knee pain and knee function;it is not recommended for patients with patellar cartilage degeneration grade IV to achieve patellar resurfacing;(2)The incidence of anterior knee pain was higher in thepatients whose lateral patellar cartilage degeneration reached grade IV at12 months after surgery;(3)The postoperative functional score and pain score of patients with knee osteoarthritis who were treated with total knee arthroplasty are significantly better,and the short-term curative effect is satisfactory.
