| Objective:To investigate the change of plasma visfatin level and the relationship betweenplasma visfatin level and metabolic risk factors in patients with different metabolicfactors who subjected to ventricular remodeling. To clarify the role of visfatin inmetabolic process and ventricular remodeling.Methods:Choose126patients who subjected to ventricular remodeling in the departmentof cardiovascular and endocrine of First Affiliated Hospital of Nanchang Universityfrom Apr2011to Jan2012, the group of ventricular remodeling was divided intoessential hypertension group (n=59) and hypertension with the metabolic syndromegroup (n=67) according to risk factors; The control group was chosen the healthyexamined (n=39).The concentrations of plasma visfatin were measured with enzyme-linked immunosorbent assay (ELISA) in all cases. The biochemical indexs includingfasting plasma glucose (FPG), triglycerides (TG), total cholesterol (TC), high-densitylipoprotein (HDL), low density lipoprotein (LDL) and so on were examined byautomatic biochemistry analysis instrument. The relevant clinical indexs includingheight, weight, waist circumference (WC) and so on. The left ventricular enddiast-olic diameter (LVEDd), end-diastolic left ventricular wall thickness (LVPWT) andend-diastolic ventricular septal thickness (IVST) were measured by the dopplerechocardiography and left entricular mass index (LVMI) were calculated according tothe formula. The statistical analysis was application by SPSS17.0softw-are.Results:1.The concentrations of plasma visfatin were significantly higher in hypertensionwith the metabolic syndrome group compared with essential hypertension group andnormal control group[16.79±3.98ng/ml VS7.77±1.14ng/ml VS7.35±0.27ng/ml)],the differences are statistically significant (P<0.01).2. LVMI was significantly higher in hypertension with the metabolic syndromegroup compared with essential hypertension group(180.34±22.04g/m~2VS134.57± 24.29g/m2), the differences are statistically significant (P<0.01), LVMI wassignificantly higher in essential hypertension group compared with normal controlgroup (134.57±24.29g/m~2VS88.89±16.14g/m~2),the differences are statistically sign-ificant (P<0.01).3. Correlation analysis showed that plasma visfatin level are positively correla-ted with waist circumference (WC), body mass index (BMI), blood pressure,triglycerides(TG)and fasting plasma glucose (FPG).4. Multiple stepwise regression analysis showed that plasma visfatin levels(VS),diastolic blood pressure(DBP), fasting plasma glucose(FPG), ejection fraction(EF),waist circumference (WC) are risk factors of LVMI.Conclusion:Visfatin is the risk factors of metabolic syndrome and the caused by one of theinitiating factor of ventricular remodeling.The plasma visfatin level and left ventricu-lar mass index in patients with ventricular remodeling increased as metabolic riskfactors increased. The influence of visfatin and hypertension on ventricular remodeli-ng aggravated collaboratively rather than simply added. Visfatin is the risk factorwhich caused the metabolic syndrome. |
