Objective:1. To establish the pancreaticoduodenal transplantation (PDT) model inthe rat with microsurgery technique, so as to lay the foundation for furtherPDT relate researches.2. To investigate whether NS398(the specific cyclooxygenase-2inhibitor) can inhibit ischemia-reperfusion injury (I-RI) followingpancreaticoduodenal transplantation in rats, and the influence of P-selectin(Ps) and intercellular adhesion molecule-1(ICAM-1).Methods:1. The diabetes mellitus Wistar rats induced by Streptozotocin (STZ)were recipients, and pancreatic allograft from SD rats was transplanted toWistar recipients, which was performed in40rats. The Wistar rats wereoperated without clamping inferior vena cava and aorta ventralis. The portalvein of the graft was anastomosed with the recipient’s left renal vein by cufftechnique while the aorta ventralis of the graft was anastomosed with the recipient’s left renal artery by the same technique. We determined thegeneral status of recipients and blood glucose levels of the recipients afteroperation.2.45Wistar rats and30SD rats were randomized into three groups.Group A (sham operation group):15Wistar rats; group B (transplantedgroup):15Wistar rats which were recipients and15SD rats which weredonors were matched randomly; group C (transplant with NS398group):15Wistar rats which were recipients and15SD rats which were donors werematched randomly. Blood samples in each group were harvested at1,3and6hours after opening the blood supply for serum amylase assay. Pancreassamples were harvested at the same time for morphological study using lightmicroscopy, and the expression of Ps and ICAM-1protein and gene ofpancreas were analyzed by immunohistochemical and RT-PCR method.Results:1. In the40models, there were36rats survived more than3days, andthe success rate of the operation was90%. The level of blood glucose ofsuccessful transplanted models recovered to normal in24hours after theoperation. And in the40rats, the average time of donor operation was (68±4)minutes while recipient operation was (55±4) minutes. The anastomosistime of artery was (5±1) minutes while the time of vein was (6±1) minutes.2. Observing by the light microscope: the pancreas tissue of group A ineach time spot had no changing, and the damage was aggravated in the group B, while these changes were mild in group C. The level of serum amylase:there was no changing in each time spot of group A, and it was elevatedgradually in group B, while it was mild in group C compared with group B.Ps and ICAM-1: there was no expression in each time spot of group A, andthe expression of Ps and ICAM-1in each time spot of group B and C can beobserved, but that in group C was conspicuous lower than that in group B.Conclusion:1. This model of PDT simplified the procedures of vascularanastomosis, and can establish the model of PDT in rats steadily, and lay thefoundation for further research.2. The IR-I following PDT in rats can be inhibited by NS398; andNS398can reduce the expression of Ps and ICAM-1, so it has protectiveeffects.
