Background:Chronic heart failure (chronic heart failure,CHF) is a chronic progress disease,which the basic mechanism is ventricular remodeling,along with the development ofchronic heart failure. In connmon neuroendocrine, immune and inflammatorycommon involved in CHF ventricular remodeling. It has seen researchedInflammation in CHF pathophysiological mechanism, but the mechanism is unclear.Interleukin-6、hyper-sensitive response protection C were significantly higher in hearfailure. The others agreen with inflammation factors can prediction the.future of heartfailure. Mononuclear cells is important in immune and inflammatory reaction.theactive state and relevant mechanism of peripheral blood mononuclear cells in heartfailure needs further research. Zinc finger protein of A20, the abbreviation of A20, isa kind of cytoplasm protein, which adjust inflammation and inhibiting cell apoptosis,restrain nuclear transcription factors-kB activity。 it has been reported inatherosclerosis, cancer, type1diabetes,endotoxin blood disease,organ transplantsinflammation plays an important role in the process,but the l mechanism ofPathological mechanism in heart failure is unknown. Understand inflammationactivate condition in CHF patients and the of A20what can further clarify thepathogenesis of CHF and provided the clinical evidence.Objective:To measure expression of zinc finger protein A20in peripheral bloodmononuclear cells(PBMC)、secret of IL-6and plasma of hs-CRP、BNP from patientswith chronic heart failure, Explore the function and meaning. of zinc finger protein inCHF Methods:There are40patients with chronic heart failure from department of ChangshaCentral Hospital from October,2011to February2012.Select40patients who the ageand sex are matching with CHF patients from Medical branch. All patients collectedperipheral blood after fasting eight hours in the morning, used the way of densitygradient centrifugation(Ficoll method) Separating out a single nuclear cells In37℃,carbon dioxide temperature box in the training four hours and then collectedsecretion by peripheral blood monocytes after fixed with4%Polyphosphate ofmaldehyde in Training hole,single nuclear cells that put in refrigerator of thetemperature in-20℃.Immune chemical methods detection A20zinc finger protein,enzyme-linked immunosorbent assays test interleukin-6andhs-CRP,radioimmunoassay examine BNPResults:1、In contrast to the control group,the indicator of gender, age, smoking, bloodpressure, blood sugar and cholesterol and triglycerides, high-density lipoproteincholesterol which the difference was statistically insignificant,but total cholesteroland low density lipoprotein cholesterol significantly higher than those in the controlgroup[(5.58±1.57)mmol/L vs(4.76±0.81)mmol/L,(3.16±1.02) mmol/L vs(2.44±0.65) mmol/L,all p﹤0.05)],the difference was statistically significant。2、the heart failure group had more higher concentration of BNP[(875±11.67)vs (16±0.74)P<0.05)]the difference was statistically significant, the differencebetween in cardiac function was statisticallysignificant[(Ⅱgrade(235±98.23)pg/ml、 Ⅲgrade(459pg/ml±188.26)pg/ml、Ⅳgrade(1019pg/ml±308.46)pg/ml,p﹤0.05]3、Compared with patients of control group, hs-CRP concentration in Plasmaincreased dramatically [(10.08±4.28)mg/l vs (4.29±1.11) mg/l p﹤0.05)]4、Compared with patients of control group,IL-6concentration in secretionsby peripheral blood monocytes increased dramatically.[(30.1±12.28)pg/ml vs(2.4±1.5)pg/ml](p﹤0.05) 5、Compared with patients of control group,A20concentration in peripheralblood monocytes increased dramatically[(7.78±3.42)%vs (0.63±0.32)%,p﹤0.05)]6、The levels of A20in peripheral blood monocytes was positively correlatedwith the plasma levels of hs-CRP7、The levels of A20in peripheral blood monocytes was positively correlatedwith the secretions of IL-68、The levels of A20in peripheral blood monocytes was positively correlatedwith the plasma levels of BNP,the difference between in cardiac function wasstatistically significant,[Ⅱgrade(0.97±0.25)%;Ⅲgrade(16.81±5.9)%;Ⅳgrade(29.23±11.7)%](all p﹤0.05)Conclusions:1、The inflammation activated which existing in the peripheral blood of Chronicheart failure patient, monocytes activation may is one of the important causes result2、The finger protein of A20root in Peripheral blood mononuclear cells involvedin chronic heart failure process of inflammation, and the cardiac function related withthe degree of damage closely.
